【摘要】 目的　观察高血压病左心室肥厚患者血浆脑钠素 (BNP)水平 ,并血管紧张素转换酶抑制剂 (ACEI)治疗前后左心室肥厚的逆转情况 ,探讨血浆BNP水平可否作为判断左心室肥厚与逆转的临床指标。方法　筛选高血压病伴左心室肥厚组患者 4 0例 ,高血压病无左心室肥厚组患者 30例及正常血压对照组 30例 ,予以高血压病患者喹那普利 5～ 10mg治疗 2 4周 ,采用超声心动图测定左心室质量指数 (LVMI) ,E/A比值 ;放射免疫分析法测定血浆BNP水平 ,对BNP与LVMI、E/A比值作直线相关分析。结果　　高血压病左心室肥厚患者组的血浆BNP水平明显高于高血压病无左心室肥厚患者组和正常血压对照组 (P <0 0 1) ,经ACEI治疗后高血压病左心室肥厚患者组血浆BNP水平明显下降 (P <0 0 1) ,且与LVMI呈显著正相关 (治疗前r =0 4 5 ,P <0 0 1;治疗后r =0 39,P<0 0 1) ,与E/A比值呈显著负相关 (治疗前r =- 0 70 ,P <0 0 1;治疗后r =- 0 6 9,P <0 0 1)。结论　血浆BNP水平可作为判断高血压病左心室肥厚与逆转的一个敏感指标。更多还原

【Abstract】 Objective To investigate the level of plasma brain nat riuratic peptide (BNP) in essential hypertensive patients with left ventricular hypertrophy before and after treatment with ACEI in attempt to identify the role of the plasma BNP in the reversal of left ventricular hypertrophy. Me theds Two dimensional echocardiography was performed to determine left ventricular mass index (LVMI). Plasma BNP was measured by RIA in 40 essential hypertensive patients with left ventricular hypertrophy (LVH). Thirty essential hypertensive patients without LVH and 30 healthy subjects served as control. H ypertensive patients with LVH were treated with quirapril 5-10 mg for 24 weeks. The correlations between plasma BNP and LVMI, ratio of E/A were analyzed. Results Plasma BNP was significantly increased (P<001) in hy pertensive patients with LVH as compared with those without LVH and healthy subj ects \[(195.9±58.4)ng/L vs EH without LVH (902±23.5)ng/L and healthy (35.1± 15.3)ng/L, P<001\]. Quirapril treatment significantly decreased plasma BN P in patient with and without LVH \[with LVH: from (902±23.5)ng/L to (74.9± 205)ng/L, without LVH: from (195.9±58.4)ng/L to (63.2±23.1)ng/L\] (P<0 01). The plasma BNP positively correlated with LVMI in hypertensive patients and negatively correlated with ratio of E/A in hypertensive patients. C onclusion The plasma BNP may be a marker for existence and reversal of left ventricular hypertrophy.更多还原
