多发性硬化周围神经病变的电生理评价
Peripheral neuropathy in multiple sclerosis:electrophysiological evaluation
【摘要】 目的 研究多发性硬化 (MS)患者的周围神经病变 ,并评价神经电生理技术的应用价值。方法 采用神经传导速度 (NCV)技术检测MS患者周围神经的运动传导速度 (MCV)、感觉传导速度 (SCV)及其潜伏期 ;采用运动诱发电位 (MEP)检测正中神经和胫神经的潜伏期 ;采用F波检测正中神经的出现率和传导速度。结果 MS患者NCV均不同程度地减慢 ,MCV的异常率高于SCV ,NCV结果提示轴突损害比脱髓鞘显著。MEP测得肘点和腰 4点的潜伏期延长 ,提示正中神经远端和腰骶神经根功能的损害。部分患者F波的出现率降低 ,提示周围神经根功能异常。结论 MS患者存在周围神经病变 ;综合运用电生理技术可以全面地评价MS周围神经功能。
【Abstract】 Objective To study peripheral neuropathy in multiple sclerosis (MS) patients,and to evaluate the applying value of the electroneurophysiology techniques. Methods By using of nerve conduction velocity (NCV) technique,motor conduction velocity (MCV),sensory conduction velocity (SCV) and latency of peripheral nerves were examined;by using of motor evoked potential (MEP),latencies of median and tibial nerves were examined;by using of F-wave,the percentage of responses and conduction velocity of median nerves were examined. Results MCV and SCV of MS patients were slow in different degree;the abnormality percentage of MCV was higher than the one of SCV. The NCV results indicated that axonal damages were more significant than demyelination. The MEP prolonged latencies of elbow and L4 sites indicated injuries in the distal segment of median nerve and in lumbar and sacral roots. The decreased percentages of F-wave responses indicate peripheral radiculopathy in some patients. Conclusion There are peripheral neuropathies of MS patients. It is sound to evaluate the function on peripheral nerves of MS by using combined techniques of electroneurophysiology.
