CT测量脑中线移位预测脑疝

【摘要】 目的 探讨CT测量脑中线移位预测脑疝的计分方法。方法 对临界脑疝组、脑疝组及对照组共458例脑外伤、脑出血病例,进行脑中线CT测量和统计学分析,寻找出脑中线各结构移位的脑疝危险系数,并将危险系数分级量化进行“脑疝预测计分”,对44例脑外伤、脑出血病例前瞻性计分评估。结果 脑中线各结构移位的脑疝危险系数是:中脑移位3mm,大脑镰移位6mm,三脑室移位9mm,透明隔移位12mm;“脑疝预测计分法”计分为6分时即为脑疝临界状态,分值增加危险性加大。结论 “脑疝预测计分法”是CT预测脑疝的一种快捷、简便、有效的方法。

【Abstract】 Objective To assess the method of forecasting brain hernia in CT images by measuring brain midline. Methods 458 cases of intracranial hemorrhage and brain trauma were divided into critical brain hernia group, brain hernia group and contrast group. Each case was measured the brain midline and was analyzed by statistics, then we found out the danger coefficient of brain hernia, and the danger coefficient was been in quantitative grade and analyzed by the scoring of forecasting brain hernia. Using the method, we estimated 44 cases of in tracranial hemorrhage and brain trauma. Results The danger coefficient of brain hernia of each case of brain midline structures displacement is following: midbrain displacement (3mm), cerebral falx displacement (6mm), third ventricle displacement (9mm), septum pellucidum displacement (12mm). The critical state of brain hernia is 6 score which worked out by the scoring of forecasting brain hernia, the score is advanced and the perniciousness is turning up. Conclusion The method of forecasting brain hernia in CT images by measuring brain midline is a kind of rapid, uncomplicated and very effective.
