【摘要】 目的:了解药物变态反应的发病及诊治情况。方法:对近年来诊断明确、资料齐全的药物变态反应30例作问顾分析。结果:小儿16例,年龄5天-12岁;成人14例,年龄15-65岁。临床表现仅为发热8例,发热伴皮疹18例,发热、皮疹伴药物性肺炎4例。发热表现驰张热13例,稽留热9例,不规则发热6例,低热2例,其中长期发热4例,伴相对缓脉4例;血清谷丙转氨酶升高8例,血白细胞及嗜酸性料细胞升高各9例。药物变态反应相关药物中抗生素居多(26／30,86.6％),停药或和辅助糖皮质激素治疗均恢复正常。结论:药物变态反应可发生任何年龄组,可表现仅为发热,以至长期发热,或伴皮疹、药物性肺炎等,停药或和辅以糖皮质激素治疗可恢复正常。本组药物变态反应相关药物中抗生素居多,因此应严格其使用指征,以减少药物变态反应的发生。更多还原

【Abstract】 Background: To understand the epidemiological and clinical characteristics of drug allergy. Methods: A retrospective clinical analysis on 30 cases of drug allergy diagnosed during recent years were performed. Results: In all the 30 cases, 16 of them are age betseen 5 days to 12 years, the rest 14 age range from 15 to 65 years. In all cases, fever was the single clinical manifestation in 8 patients, fever accompanied by skin eruptions in 18 cases, 4 cases were found to have fever accompanied by skin eruption and drug induced pnuemonia. The types of fever included remittent fever (13 cases), continued fever (9 cases), irregular fever (6 cases) and low fever (2 cases). In all patients, 4 cases were found fever, accommpanied with long term fever. 4 cases were found with relative bradycardia. Higher Ievel of serum glutamic pyruvic transaminase(GPT) were seen in 8 cases. Higher Ievel of peripheral blood leucocyte and eosinophilic granulocyte were found in 9 cases respectivly. The major part of drugs found related to drug allergy are antibiotics which were 26 in 30 and makes up to 86.6%. Cease of intake of drugs(withdraw ) or assisted with therapy of glucocorticoid could heal the fever. Conclusion: Drug allergy could be seen in any age. It could manifestate as fever only, or long-term fever, even with skin eruption and drug-induced pnuemonia. Cease of intake of drugs or (with) assisted therapy of glucocorticoid could heal the fever. In our observation, antibiotics were found majorly involved in drug allergy. Thus, indication of antibiotic usage should be strictly controlled to reduce incidentce rate of drug allergy.更多还原
