年龄对丙泊酚药效学的影响
The Influence of Age on Pharmacodynamics of Propofol
【摘要】 目的 :确定以MAP作为效应指标时 ,年龄对丙泊酚效应位的消除速率常数 (ke0 )和 5 0 %最大效应的效应位浓度 (EC50 )的影响。方法 :15例病人按年龄分为老年组 (>6 5岁 ,n =8)和青壮年组 (<6 5岁 ,n =7) ,分别持续输注丙泊酚 6 0mL/h和 75mL/h直至病人意识消失。持续监测MAP直至两者恢复至用药前 95 %以上水平。效应室消除速率常数ke0根据Hill曲线计算。结果 :老年人和青壮年人的EC50 分别为 2 4 9μg/mL和5 32 μg/mL(P <0 0 5 ) ;ke0分别是 0 14 5min-1和 0 193min-1(P <0 0 5 )。结论 :年龄显著影响ke0和EC50 。持续输注丙泊酚对老年病人MAP的影响大且恢复缓慢
【Abstract】 Objective: To determine the influence of age on ke0 and EC50 of propofol when MAP was regarded as the index of effect. Methods: 15 patients were divided into the young and middle-aged group (<65years old, n=7) and the elderly group (>years old, n=8), received continuous infusion of propofol 75mL/h and 60mL/h respectively up to loss of consciousness. MAP was recorded continuously from start of infusion to restore MAP to 95% of premedication. ke0 and EC50 were estimated by fitting a Hill curve. Results: The EC50 of MAP were 2.49μg/mL and 5.32μg/mL in elderly group and young and middle-aged group respectively (P<0.05). The ke0s were 0.145min-1 and 0.193min-1 in elderly group and young and middle-aged group respectively (P<0.05).Conclusion: Age has significant effect on ke0 and EC50 of MAP. Continuous infusion of propofol may decrease MAP to a greater degree and recover slowly in elderly.
