眶上裂外侧区和中央区的手术应用显微解剖

【摘要】 目的 为眶上裂区手术提供应用显微解剖学基础。方法 观测 3 0例 ( 60眼 )成人头颅干标本眶上裂骨性结构 ;在手术显微镜下观测 2 0例 ( 4 0眼 )成人头颅湿标本眶上裂区穿行的神经、血管位置及毗邻关系。结果 眶上裂外壁上、下半边夹角为 14 4 2 7°± 2 0 0 3° ;滑车神经和动眼神经上支距眶上裂上壁下缘分别为 ( 0 3 5± 0 12 )mm和 ( 0 3 9± 0 2 2 )mm ;眼上静脉颅中窝段具有静脉窦结构。结论 眶上裂穿行结构的位置恒定 ,对该区外伤及肿瘤的手术诊治有参考意义。

【Abstract】 Objective To provide the applied microanatomical basis for the operation of the superior orbital fissure(SOF).Methods The osseous structures of SOF were observed in 30 cases(60 eyes) of dry adult cadaveric heads;The course and relationship of nerves and vascular structures passing through the lateral and central sector of SOF were studied in 20 cases(40 eyes) of wet adult cadaveric heads under the microscope.Results The angle between the upper and lower segment of the lateral edge of SOF was 144.27°±20.03°;The trochlear nerve and the superior division of oculomotor nerve were (0.35±0.12)mm and (0.39±0.22)mm inferior to the upper margin of SOF respectively;The middel fossa segment of superior ophthalmic vein had the structure of venous sinus.Conclusion The position of the structures passing through the SOF is constant,which provides reference for the diagnosis and treatment of the trauma and tumors of SOF.
