机械通气联合支气管肺泡灌洗治疗重症哮喘

【摘要】 目的 探讨机械通气必要时联合支气管肺泡灌洗 (BAL)在治疗重症哮喘中的作用。方法 选择常规治疗后病情缓解不明显的重症哮喘呼吸衰竭患者 13例 ,建立人工气道行机械通气治疗 ,对 2 4h后气道压力仍偏高者 ,应用生理盐水经纤支镜行BAL ,清除呼吸道痰栓。结果  13例患者中 ,8例通气 2 4h后 ,另 5例行BAL 2 4h内气道阻力逐渐下降 ,临床症状好转 ,缺O2 及CO2 潴留得到改善。 12例患者成功脱机出院。结论 在机械通气基础上联合BAL是治疗重症哮喘的有效措施

【Abstract】 Objective To discuss the roles of mechanical ventilation associated with bronchoalveolar lavage when necessary, in treating severe asthma.Methods 13 patients no obvious remission withsevere asthma respiratory failure after routine treatment, create man-made airway to enforce mechanical ventilation, and perform bronchoalveolar lavage by fibrobronchoscope with saline to clear respiratory tract sputum in the patients with high airway pressure after 24 hours. Results In 8 patients of 13 after mechanical ventilation, and 5 patients after bronchoalveolar lavage, the airway resistance descend gradually; clinical symptom improve ; anoxia and carbon dioxide retention ameliorate after 24 hours. 12 patients discharge after successfully detach with breathing machine. Conclusions Mechanical ventilation associated with bronchoalveolar lavage are utility measures to treat severe asthma.
