多波长氪离子激光治疗糖尿病视网膜病变疗效观察

【摘要】 目的观察多波长氪离子激光对116例(182眼)糖尿病视网膜病变(DR)的疗效。方法根据患者病变性质、部位、分期和屈光间质情况分组,选择不同波长激光进行不同方法视网膜光凝。局部光凝组124眼,全视网膜光凝组58眼,术前均行视力、检眼镜、裂隙灯显微镜加三面镜和荧光素眼底血管造影检查,术后随访1~1.5年。结果局部光凝组病变较轻40眼,病变较重84眼,治疗1次后随访显示二者疗效有显著性差异(P<0.05)。全视网膜光凝组,其中增生前期DR26眼,光凝后有效22眼(85%)、无效4眼(15%)。增生期DR32眼,光凝后有效20眼(63%)、无效12眼(37%)。结论多波长氪离子激光是治疗DR的有效方法,依据病变情况使用不同波长扩大了治疗DR的适应证。早期进行局部光凝治疗DR可减少治疗次数和并发症。

【Abstract】 Objective To investigate the therapeutic outcom e of different wavelength krypton lasers on diabetic retinopathy(DR).Methods One hundred eighty-two eyes(116cases)with diabetic retinopathy were trea ted using different wavelength krypton lasers according to their pathological p roperties,location and different stages.Local photocoagulation was taken for 124eyes,and panretinal laser photocoagulation for58eyes.The visual acuity ,vitreous body,lens and retina were examined pre-operatively by slit-lamp m icroscopy,ophthalmoscopy and fundus fluorescein angiography.All patients were followed up for at least12months after krypton laser treatment.Results Forty eyes with mild and84with severe retinopathy in124eyes treaded by local phot ocoagulation showed different therapeutic outcome (P<0.05).In58eyes t reated by panretinal laser photocoagulation,22(85%)showed a good effecti veness and4(15%)non-effectiveness in26eyes with non-proliferation DR, 20(63%)showed effective and12(36%)uneffective in20eyes with prol iferation DR.Conclusion Different wavelength of krypton lasers photocoagulatio n is an effective method in the treatment of diabetic retinopathy,and these re sult suggests earlier focal photocoagulation is benefit for the eyes with DR.
