宝石加工工人矽肺致残程度的研究

【摘要】 目的 探讨宝石加工工人矽肺患者病残的特征及尘肺致残程度鉴定标准的可行性。方法 按国家现行《职工工伤与职业病致残程度鉴定》标准 ,对 10 5例宝石加工工人矽肺患者进行病残程度的鉴定。结果  10 5例患者中 ,2级病残 17例 (16 19% )、3级 17例 (16 19% )、4级 3 3例 (3 1 43 % )、6级 10例 (9 5 2 % )、7级 2 8例 (2 6 67% )。矽肺期别、小阴影总密集度、肺功能损伤、呼吸困难、血气指标异常与致残程度之间有相关关系 (P <0 0 1或P <0 0 5 )。矽肺期别、肺功能损伤、呼吸困难、血气指标异常和并发活动性肺结核决定病残等级的分别为 64例 (60 95 % )、2 0例 (19 0 5 % )、3例(2 86% )、9例 (8 5 7% )和 9例 (8 5 7% )。呼吸困难与肺功能损伤的符合率为 49 5 2 %。结论 宝石加工工人矽肺患者病残程度较重 ,其病残程度随矽肺期别和肺功能损伤等级增加而增高。尘肺致残程度鉴定标准是实用可行的 ,但个别地方建议修改。

【Abstract】 Objective To study the features of the grad at ion of disability caused by gem worker’s silicosis (GWS) and the feasibility of the standard on the evaluation of disability caused by pneumoconiosis. M ethods 105 cases GWS were assessed on their work-capacity based on the standard of assessment and gradation of disability caused by work-related inju ries and occupational diseases. Results Among 105 cases of GWS ,2,3,4,6,7 gradation of disability caused by GWS were 16.19%,16.19%,31.43%,9 .52% and 26.67% respectively. There were significant correlation differences b e tween the gradation of disability and stage of GWS profusion of the small round opacities, damage of lung function, degree of shortness of breath, abnormal inde xes of blood-gas analysis (P<0.01 or P<0.05). The gradation of disabi lity determined by the stage of GWS, damage of lung function, shortness of breat h, abnormal indexes of blood-gas analysis and complication of active pulmonary tuberculosis were 60.95%, 19.05%, 2.86%, 8.57% and 8.57% respectively. 49. 5 2% of the shortness of breath coincided with the damage of lung function. Conclusion The gradation of disability caused by GWS was raised with the increasing stage of GWS and the damage of lung function. The assessment stan dard for disability caused by pneumoconiosis was feasible, but should be amended to some extent.
