先天性输尿管不全梗阻的X线与B超表现分析

【摘要】 目的 :探讨先天性输尿管不全梗阻的X线与B超表现 ,提高对本病的认识水平。方法 :回顾性分析 2 4例先天性输尿管不全梗阻的X线B超检查资料。全部病例均经静脉尿路造影 (intravenousurograghy ,IVU) ,8例行逆行尿路造影(retrograderuograghy ,RUG) ,4例行经皮肾穿刺造影 ,3例行经肾造瘘造影 ;2 4例均行B超检查。结果 :2 4例均经手术及病理证实 ,输尿管狭窄 16例 ;迷走血管压迫 3例 ,1例伴肾旋转不良 ;纤维束带粘连、牵拉和压迫 3例 ;输尿管瓣膜形成 2例。泌尿系造影诊断符合率为 80 7%、B超诊断符合率为 5 8.3% ,两者互补诊断符合率为 10 0 %。结论 :静脉尿路造影、逆行尿路造影、经皮肾穿刺造影和B超检查优势相互弥补 ,可对此病做出较确切诊断。

【Abstract】 Objective:To explore the value of X-ray and B-ultrasound in diagnosis of congenital ureter obstruction in order to improve the recognition of the disorder.Methods:The data of 24 cases with congenital ureter obstruction diagnosed with X-ray and B-ultrasound were analyzed retrospectively. The diagnosis of the disease was confirmed by intravenous urography (IVU) in all the 24 cases, retrograde urography (RUG) in 8 and percutaneous trans-renal puncture in 4. All the cases in this group were also examined by B-ultrasound.Results:The main causes for the disease included ureteric stenosis in 16 cases, pressure by vagrant vessel in 3, renal mal-rotation in one, adherence, attraction and pressure of fibrous band in 3, and formation of ureteric valve in 2. The agreement of urography in diagnosis of the disease was 80.7%. The agreement of B-ultrasound in the diagnosis of the disease was 58.3%. When both urography and B-ultrasound were used,the agreement diagnosis of the disease was 100%. All cases were confirmed by operation and pathological examination.Conclusion:IVU,RUG and percutaneous trans-renal puncture are common and useful methods in the diagnosis of congenital ureter obstruction. If both X-ray and B-ultrasound exams are used, generally, the diagnosis can be made correctly.
