【摘要】 目的 :介绍iPath 2 0 0介入性磁共振引导下进行神经根阻滞的技术操作方法 ,并评价其准确性。方法 :13例椎间盘突出的病人 ,均在iPath 2 0 0介入性磁共振引导下进行神经根阻滞。 4例为颈椎神经根 ,9例腰椎神经根。所用设备为Philips公司生产的开放式 0 .2 3T磁共振检查仪 (Proview) ,并配有iPath2 0 0光学引导系统。穿刺针采用德国Daum公司的磁共振兼容性 2 0G穿刺针。所有病人术后 1~ 2个月 (平均 1.2个月 )就神经根痛的疗效进行临床体检。神经根痛的疗效分为 4级 :①明显缓解 =疼痛明显或完全缓解。②暂时缓解 =疼痛暂时缓解。③完全无效但疼痛无恶化。④恶化。结果 :本组病人明显缓解率为 92 .3% ( 12 /13) ,暂时缓解率为 7.7% ( 1/13)。未有无效及恶化病人。结论 :介入性磁共振引导下的神经根阻滞是一种安全、准确的技术。

【Abstract】 Objective:To describe the procedure and evaluate the accuracy of the interventional MRI guided nerve root infiltration.Methods:13 patients with intervertebral disk herniation underwent interventional MRI guided nerve root infiltration.4 cases of cervical and 9 cases of lumbar nerve roots were infiltration.All patients were performed under 0.23 T open MRI system (Proview,Philips Medical System) with optical tracking system (iPath 200,Philips Medical System).The needle was MR compatible (Daum, Germany, 20G). The outcome was evaluated as effect on radiculalgia 1~2 months (mean 1.2 months) after the procedure by clinical examination.The effect on radiculalgia was graded as follows:① Obvious relief=the pain obvious or complete relief at the time of outcome evaluation.② Temporary relief=temporary relief of pain.③ No relief but no worsening of pain.④ Worsening of pain.Results:The rate of obvious relief was 92.3% (12/13).The rate of temporary relief was 7.7% (1/13).No patient was no relief of pain or worsening.Conclusion:IMRI guided nerve root infiltration is an accurate and safe technique.
