【摘要】 目的 :探讨双相气道内正压 (BiPAP)呼吸机治疗重症哮喘的疗效。方法 :将60例急性发作期重症哮喘患者为两组 ,对照组30例先给予5 %葡萄糖40ml加氨茶碱0.25g,每日1次静注,继之以5 %葡萄糖500ml加氨茶碱0.5g加地塞米松10mg,每日1次静滴 (滴速30滴/分 ) ,博利康尼2.5mg,每日3次口服并辅以一般治疗。治疗组30例在应用上述治疗外另加予BiPAP呼吸机通气治疗 ,每日2次 ,每次2小时 ,5天为一疗程。随时观察患者的临床表现 ,并于每日治疗前后即刻分别测定最大呼气流速 (PEF) ,以治疗后PEF较入院当天用药前增加15 %以上为有效。结果 :对照组用药36～72小时起效8例 ,72小时以上起效16例 ,有效率80 % (24/30) ,无效6例。治疗组用药12～72小时起效22例 ,72小时以上起效7例 ,有效率96.67 %(29/30) ,无效1例 ,治疗组有效率显著高于对照组 (P<0.05)。结论 :在常规治疗的基础上合用BiPAP呼吸机治疗,可提高治疗重症哮喘的疗效更多还原

【Abstract】 Objective:To investigate the curative effect of biphasic intra-airway positive air pressure(BiPAP)respiratory machine in treating the severe asthma.Methods:60cases of severe asthma in acute period were divided into two groups.30cases in the control group were given with aminophylline0.25g in40ml of5%glucose solution per day by intravenous drip and then aminophylline0.5g plus dexamethasone10mg in500ml of glucose solution was used with the same method,and oral brycanyl2.5mg tid.The other30cases in the treatment group,except for the foregoing treatment,were ventilated with BiPAP respiratory machine,two hours a time bid,for5days as a course of treatment.The clinical manifestations were observed at all times.The effectiveness was determined as increasing15%of PEF after ventilation.Results:In the control group,8cases were effective in36～72hours,16were effective in more than72hours,4ineffective.The effective rate was80%(24/30).In the treatment group,22cases were effective in12～72hours,7were effective in more than72hours,1ineffective.The effective rate was96.67%(29/30),which was significantly higher than that in the control group(P<0.05).Conclusion:Using BiPAP respiratory machine based on routine treatment can raise the curative effect of severe asthma.更多还原
