“镫井喷”的术前影像学预测

【摘要】 目的 探讨内耳手术并发症“镫井喷”的术前预测方法。方法 对147例涉及内耳手术者,术前进行颞骨HRCT与MRI检查,观察内耳与颅内有无异常开放相通的腔隙。结果 147例涉及内耳手术患者中,4例可见内听道底骨性缺损,术中均有脑脊液自缺损区喷出,发生“镫井喷”现象。结论 “镫井喷”多为内听道底骨性缺损区与内耳直接相通所致,术前认真的影像学检查可以对此作出预测。

【Abstract】 Objective To explore a procedure useful to predict preoperatively the complication of stapedial gusher potentially occurring during the operation on or involving in the inner ear. Methods 147 cases operated on with a surgery involved in the inner ear were included in this study. They were all received high resolution computed tomography (HRCT) and MRI to the temporal bone at axial and coronal position and in 1 mm thickness. The abnormally opening and connecting canal potentially present between the inner ear and cranium was assayed carefully on the films. Then, the fact of this was kept under consideration with the finding during operation. Results Among these cases studied, 4 were found with bony defect in the internal acoustic meatus. They were all suffered the complication of stapedial gusher during the operation. Conclusion The bony defect in the fundus of internal acoustic meatus leading to a connecting canal present between the inner ear and subarachnoid cavity is most responsible for the occurrence of stapedial gusher as a complication during the operation involving the inner ear. This possibility can be predicted preoperatively via high qualitative image scanning and careful evaluation on the films.
