巩膜隧道切口白内障超声乳化术后角膜地形图的动态变化
Dynamical changes of corneal topograph after ultrasonic phacoemulsification with sclera tunnel incision
【摘要】 目的 为了解巩膜隧道切口超声乳化术后早期切口对全角膜弯曲度的影响。方法 白内障患者 81眼行巩膜隧道切口超声乳化术 ,观察术前及术后 1d、1周、1月的角膜地形图。结果 术后角膜地形图第 1天均呈现切口相应部位角膜变平坦 ,图形颜色向冷色变化 ,至术后 1月大部分图形与术前相比无明显差别 ,术前及术后 1d、1周、1月角膜表面规则指数 (surfaceregularityindex ,SRI)分别为 1.2 4±0 39、1.37± 0 .37、1 31± 0 .34、1.2 6± 0 .32 ,角膜表面不规则指数 (surfaceasymmetryindex ,SAI)分别为 0 .94± 0 .5 7、1.19± 0 .5 9、1.0 2± 0 6 1、0 .87± 0 .5 5 ,模拟角膜镜读数 (Simulatedkeratoscopereading ,simk)分别为 1.2 7± 0 .2 7、1.6 8± 0 78、1 2 4± 0 .74、1.12± 0 .6 8,术后较术前均无显著性差异 (P >0 0 5 ) ,术后各项值呈递减趋势。结论 巩膜隧道切口超声乳化术引起的散光较小 ,角膜地形图变化较小 ,并可很快恢复至术前。可获得良好的手术疗效 ,角膜地形图可以作为全面分析白内障术后角膜前表面形态的有效手段。
【Abstract】 Objective To investigate the early post-operative effect of the incision of phacoemulsifi cation on the whole corneal curature. Methods Eighty-one eyes of cataract that was operated with ult rasonic phacoemulsification with sclera tunnel incision.The corneal topograph w as observed 1 day,1 week,1 month pre- and post-operation. Results The corneal topograph all shows that the corneal become flattened 1 day after surgery and the picture tend to cool colour.Compared wi th preoperation most of the pictures there are no significant difference one mo nth after operation.One day,1 week,1 month after surgery,the surface regularity index(SRI)was 1.24±0.39、1.37±0.37、1.31±0.34、1.26±0.32,the surface asymme try index(SAI) was 0.94±0.57、1.19±0.59、1 02±0.61、0.87±0.55,the simulate d ieratoscope reading(Simk) was 1.27±0.27、1.68±0.78、1.24±0 74、1.12±0.68,t here is no obviously difference between pre- and post-operation(P>0.05) . All index after operation tend to decrease. Conclusion Ultrasonic phacoemulsification with sclera tunnel in cision can cause less astigmatism,the corneal topograph changes a little and the corneal shape may recover as same as pre-operation.Corneal topograph can be th e effect method for analysing the corneal shape changes after operation.
