钝性分离及镊撕法联合丝裂霉素C治疗原发性翼状胬肉
Blunt separation and ripping by tweezers combined with mitomycin C application for primary pterygium
【摘要】 目的 比较 2种手术方法治疗原发性翼状胬肉对角膜创面上皮修复及其复发率的影响。方法 对 91例 10 2眼原发性翼状胬肉随机分为A组 (钝性分离及镊撕法 ) 4 6例 5 2眼 ,B组 (常规单纯切除法 ) 4 5例 5 0眼。 2组手术均在显微镜下进行 ,术中放置同等时间、同样浓度的丝裂霉素C棉片 ,复诊时用 10 g·L-1荧光素钠染色法来观测角膜创面上皮修复所需时间及翼状胬肉的复发情况。随访 8~ 2 4个月 ,平均 14个月。结果 A、B 2组角膜创面角膜上皮修复平均时间分别为 7.2 5和 9.5d ,2组比较差异有显著意义 (P <0 0 5 ) ;随访观察A组5 2眼有 3眼复发 ,B组 5 0眼有 8眼复发 ,2组复发率比较差异有显著意义 (χ2 =2 77,P <0 0 5 )。结论钝性分离及镊撕法联合丝裂霉素C治疗原发性翼状胬肉 ,操作简便、安全性高 ,可使手术创伤降低到最低程度 ,从而降低原发性翼状胬肉术后复发率。
【Abstract】 Objective To compare the effects on the epithelial healing of corneal wound and the recurrent rate of two surgical methods for primary pterygium.Methods Ninty-one cases (102 eyes) of primary pterygium were randomly divided into group A and B. The procedures of blunt separation and ripping by tweezers was done in group A and simple excision in group B.Mitomycin C(MMC) with the same concentration and application time was used in all eyes. All cases were followed up for 8~24 months.Results The epithelial recovering time of corneal wound of group A and B were 7.25 and 9.50 days respectively;There was significant difference between two groups(P<0.05).Three eyes in group A (52 eyes) relapsed,recurrent rate 5.77%,so did 8 eyes in group B (50 eyes),recurrent rate 16.0%;There was significant difference(χ 2=2.77,P<0.05).Conclusion To treat primary pterygium with blunt separation and ripping by tweezers combined with MMC application is safe and simple,and can reduce wound of the cornea and post-operative recurrent rate.
