多层螺旋CT三维重建技术在骨关节创伤诊断中的应用

【摘要】 目的 :评价多层螺旋CT三维重建技术在骨关节创伤中的诊断价值。 方法 :5 0例外伤患者均行CT检查和三维重建 ,分析图像表现 ,并与X片进行对照。 结果 :5 0例外伤患者中CT横断面结合多平面键 (MPR)和重建容积 (VR)显示骨折 4 3例 ;CT发现了 11例X线平片上漏诊的多骨骨折。 2 1例的SSD和VR ,从外部立体地显示骨折线的长短、形态、走向 ,骨表面碎骨片的形态、大小、空间位置及骨折的移位和关节对位状况 ;MPR图像能准确地显示骨表面和骨内部的骨折线 ,骨折的移位情况和关节内部的细节。 结论 :螺旋CT结合SSD、VR、MPR图像可以提供骨关节创伤非常有用的空间信息 ,临床应用价值大 ,有利于临床医师选择治疗方案和制订手术方案。

【Abstract】 Objective: To study three dimensional imaging of spiral CT in bone and joint traumas. Methods: Fifty cases were performed pla in film, spiral CT scan and three dimensional reconstruction. Results: Transaxial imaging, together with shade surface display(SSD), volume rendering(VR) and multiplanar reconstrudion(MPR) demonstrated bone fracture in 43 cases. In those patients, 11 cases were not found by X ray plain films. The length, shape and direction of fracture, the form, size and location of detached bone fragments and dislocation of joint were displayed by SSD and VR. Joint disruption and detached bone fragments within joint could be best viewed by MPR. Conclusion: SSD, VR and MPR of spiral CT can afford very valuable space information about bone and joint trauma. Surgeon will be benefit greatly from there usefulness as a method of choice in surgical intervention and planning.
