【摘要】 目的　探讨先天性后侧半椎体切除术治疗角形脊柱后凸畸形的适应证、手术方法及临床效果。　方法　采用该方法手术治疗角形脊柱后凸 2 5例 ,经后路暴露双侧椎板 ,自双侧分离暴露关节突外侧和横突 ,切除横突。沿椎弓根自骨膜下向前分离 ,暴露多余的半椎体 ,切除多余的一节椎弓和后侧半椎体 ,自后路行双侧弓根螺钉加压棍固定。　结果　本组无脊髓损伤、感染等术后并发症发生。2 0例随访 5～ 10年 ,远期效果好 ;3例随访 1年以上 ,矫正效果良好 ;2例术后尚未及 1年 ,正在随访中。Cobb’s角从术前平均 6 8°降至术后平均 11°。　结论　先天性后侧半椎体切除术的年龄范围为 8～2 2岁 ,年龄越大越趋向于作全脊柱截骨术。更多还原

【Abstract】 Objective To discuss the methods, indications and effects of congenital post-lateral hemivertebrectomy. Methods 25 cases of kyphosis received congenital postero-lateral hemivertebrectomy.Through the posterior approach, the bilateral lamina was separated to expose the lateral articular process and transverse process. Then the transverse process was resected. Sperating along the pedical of verterbral arch and subperiostrum, surplus hemiverterbra was removed. Then the spine was fixed with pedicle screw through the posterior approach. Results No complications occurred in all patients. 20 cases got good remote effect during the 5 to 10 years follow-up, 3 received good effect with more than 1 year follow-up, and 2 were still following up. The average Cobb’s angle decreased from 68 preoperatively to 11° postoperatively. Conclusion The adaptive age of congenital pos-ateral hemivertebrectomy is 8 to 22 years old. If the age of patient exceed this accommodation, total spinal osteotomy would be take into account rather than hemiverebrectomy.更多还原
