等渗硫酸镁作为溶媒治疗哮喘的疗效观察及护理

【摘要】 目的观察等渗硫酸镁为溶媒加入沙丁胺醇作氧气雾化吸入治疗哮喘的疗效及总结护理要点。方法2002年5~12月采用等渗硫酸镁溶液加入沙丁胺醇给予40例支气管哮喘病人氧气雾化吸入,并分别测定病人吸药前后肺功能变化,观察病人吸药前后的血压、心率和呼吸情况。结果23例病人吸药后咳嗽、胸闷、呼吸困难、肺部哮鸣音明显减轻,肺功能检查各项值均较吸药前升高(均P<0.01),且吸药前后血压、心率、呼吸无明显改变(P>0.05)。结论硫酸镁能增强茁受体激动剂对腺苷酸环化酶和Na+、K+-ATP酶的作用,能消除哮喘病人对茁受体激动剂的快速减敏,提高了哮喘病人的肺通气量,从而提高了疗效。

【Abstract】 Objective To observe the effect of aerosol inhalation with 7.5% magnesium sulfate and salbutamol in the asthma patients,and summarize the experiences in the nursing care of the therapy. Methods Forty patients with asthma who were treated in such kind of aerosol inhalation between May 2002 and December 2002 were studied. The pulmonary functions and the vital signs (such as blood pressure,heart rate and respiration) of the patients were observed before and after the drug inhalation. Results Symptoms of cough,chest distress,dyspnea and sign of wheeze were improved obviously after inhalation in twenty-three patients.Parameters of pulmonary function test after inhalation were higher than those before inhalation(P < 0.01), while the vital signs had no obvious change (P > 0.05). Conclusions Magnesium sulfate can not only improve patients’ hypomagnesemia but also enhance the effect of β-adrenergic agonist which acts on the adenylate cyclase and Na+?K+ -ATP enzyme. So the aerosol inhalation with the 7.5% magnesium sulfate and salbutamol can eliminate the rapid sensitization of asthma patients to β-adrenergic agonist. It enhanced the air flow of lung and improved the therapeutic effect.
