芪棱汤对短暂性脑缺血发作患者血液流变学的影响
Effect of qileng decoction on the hemorheology of patients with transient ischemic attack
【摘要】 目的:旨在观察中药复方芪棱汤对短暂性脑缺血发作(TIA)的临床疗效,并初步探讨其作用机制。方法:选择2003-07/2003-12暨南大学第二附属医院收治的63例TIA患者,随机分为对照组(常规治疗)和治疗组(常规加芪棱汤),疗程结束后评定临床疗效,并测定患者治疗前后的血液流变学、D二聚体、血浆纤维蛋白原(Fg)、活化部分凝血活酶时间(APTT)等变化。结果:治疗组临床疗效明显优于对照组(P<0.05),且治疗组治疗后全血黏度、红细胞聚集指数、D二聚体、纤维蛋白原值分别为(4.21±1.74)mPa/s,(1.52±0.74),(234.21±64.87)μg/L,(3.16±0.77)g/L,明显低于治疗前(6.12±1.44)mPa/s,(1.74±0.22),(276.15±63.25)μg/L,(4.56±0.89)g/L,差异有显著性意义(P<0.05);组间治疗后比较纤维蛋白原、D二聚体下降水平治疗组明显优于对照组(P<0.05)。结论:降低血黏度改善血液流变学,调整凝血纤溶活性,可能是芪棱汤治疗TIA的主要作用机制之一。
【Abstract】 AIM:To observe the clinical curative efficacy of the Chinese herb of qileng decoction on transient ischemic attack(TIA),and preliminarily probe into its mechanism. METHODS:Sixty three TIA patients,who were treated in the Second Affiliated Hospital of Jinan University,were randomly divided into control group and treatment group,all the patients received routine treatment,besides those in the treatment group were treated with qileng decoction.Clinical curative efficacy was evaluated,and the changes of hemorheology,D dimer(DD), plasma fibrinogen(Fg) and activated partial thromboplastin time(APTT) before and after treatment were measured. RESULTS:The clinical curative efficacy in the treatment group was obviously superior to that in the control group(P< 0.05),and the blood viscosity, erythrocyte aggregation index,DD and Fg in the treatment after treatment were(4.21±1.74) mPa/s,(1.52±0.74),(234.21±64.87) μg/L,(3.16±0.77) g/L,significantly lower than those before treatment[(6.12±1.44) mPa/s,(1.74±0.22),(276.15±63.25) μg/L,(4.56±0.89)g/L](P< 0.05).The levels of Fg and DD after treatment were decreased more obviously in the treatment group than in the control group(P< 0.05). CONCLUSION:Decreasing blood viscosity, ameliorating hemorheology and regulating activity of blood coagulation and fibrinolysis are the main possible mechanisms for qileng decoction in treating TIA.
