【摘要】 目的:探讨吸烟与利培酮治疗剂量、疗效和副作用的关系。方法:调查148例符合中国精神疾病诊断与分类手册第2版修订版和美国精神疾病分类和统计手册第3版的诊断标准的精神分裂症患者,其中吸烟患者78例,不吸烟患者70例。用阴性和阳性症状量表(PANSS)减分率评价临床有效性,用副反应量表(TESS)分值评价安全性。对吸烟和不吸烟患者进行利培酮治疗剂量、疗效和副作用的比较。结果:在利培酮平均日剂量相同的情况下,吸烟患者和不吸烟患者PANSS减分率分别为63.7±4.8和74.8±5.2,差异有显著性(t=13.503,P<0.01);TESS总分分别为1.70±1.05和2.62±1.12,差异有显著性(t=5.157,P<0.01)。结论:要使吸烟患者达到不吸烟患者同样的治疗效果,应使用较大剂量的利培酮。临床医生在评价精神药物副作用时,应注意到吸烟这一因素的影响。更多还原

【Abstract】 AIM:To study the relation of smoking with the dosage,therapeutic efficacy and side effects of risperidone. METHODS:Totally 148 schizophrenic inpatients who were in accordance with the diagnostic criteria of the second edition of Chinese Classification and Diagnostic Criteria of Mental Disorders(CCMD-Ⅱ-R) and American Diagnostic and Statistical Manual of Mental Disorders-third edition(DSM-Ⅲ), including 78 smokers and 70 nonsmokers,were investigated.The clinical effectiveness and safety were evaluated by the decreasing rate of score of positive and negative syndrome scale(PANSS) and value of treatment emergent symptom scale(TESS) respectively.The dosage,therapeutic efficacy and side effects of risperidone were compared between the smoking and nonsmoking patients. RESULTS:Under the same daily dosage of risperidone,the decreasing rates of PANSS score in the smoking patients(63.7±4.8) was significantly different from that in the nonsmoking patients(74.8±5.2)(t =13.503,P< 0.01).There was significant difference in the total score of TESS between the smoking and nonsmoking patients (1.70±1.05 and 2.62±1.12)(t=5.157,P< 0.01). CONCLUSION:Risperidone of large dosage should be used in the smoking patients so as to obtain the same therapeutic efficacy as the nonsmoking patients.Clinical physicians should pay attention to the influence of smoking while evaluating the side effects of psychotropic drug.更多还原
