原发性高血压患者血浆神经肽Y、神经降压素水平与心功能损害及其预后的相关性(英文)

【摘要】 背景:研究证实神经肽Y和神经降压素(neurotensin,NT)具有重要的血管调节作用,与原发性高血压的发生发展有密切关系,而其与靶器官损害及其预后的关系有待进一步研究。目的:探讨原发性高血压病患者血浆神经肽Y及NT水平的变化,以及与高血压性心功能损害的关系。设计:以诊断为依据的横断面研究。地点、对象和方法:选择符合原发性高血压诊断标准的解放军第304医院住院患者80例,28例健康查体者或健康志愿者作为对照。用放射免疫分析法检测血浆神经肽Y,NT浓度,同时检查患者心脏结构和功能。主要观察指标:按原发性高血压病分期、心功能分级、心脏左室后壁和室间隔厚度分类后测定的血浆神经肽Y,NT浓度。结果:原发性高血压Ⅰ,Ⅱ,Ⅲ期3组患者血浆神经肽Y浓度分别为(101±31),(153±41),(198±46)ng/L,差异有显著性意义(P=0.0001);3组患者血浆NT水平分别为(96±32),(78±31),(54±24)ng/L,差异有显著性意义(P=0.0001)。原发性高血压病患者按心功能分级分为1,2,3级3组,神经肽Y浓度分别为(112±34),(159±33),(216±40)ng/L,差异有显著性意义(P=0.0001);3组NT浓度分别为(99±27),(68±30),(48±15)ng/L,差异有显著性意义(P=0.0001),随心功能恶化,神经肽Y水平升高,而NT水平降低。原发性高血压患者伴有室间隔和(或)左室?

【Abstract】 BACKGROUND:It has been proved that neuropeptide Y and neurotensin(NT) play an important role in the vessel modulation,and have a close relation with the occu rrence and development of essential hypertention.However,its link with the targe t organ damage and the prognosis deserves further studies. OBJECTIVE:To discuss the changes in plasma neuropeptide Y and NT level in esse ntial hypertension patients as well as its connection with hypertensive cardiac functional damage. DESIGN:Cross sectional study based on diagnosis. SETTING,PARTICIPANTS and METHODS:A total of 80 patients admitted to the 304 Ho spital of PLA were chosen according to Essential Hypertension Diagnosis Standard ,and another 28 people who had been checked up as healthy or healthy volunteers were taken as the controls.The levels of plasma neuropeptide Y and NT were measu red by radioimmunoassay,and cardiac structure and function were examined at the same time. MAIN OUTCOME MEASURES:Plasma neuropeptide Y and NT were measured after classif ication of essential hypertension staging,cardiac function grading, and thicknes s of posterior wall of left ventricle and interventricular septum. RESULTS:The level of plasma neuropeptide Y in the patients with essential hype rtension of three periods(stagesⅠ,Ⅱ,Ⅲ) was(101±31),(153±41), and(198 ±46)ng/L,respectively,with significant differences(P=0.000 1).Likewise,NT lev el in the 3 groups was(96±32),(78±31),(54±24)ng/L ,respectively,also w ith significant differences(P=0.000 1).The patients with essential hypertension were divided into three groups(grades 1,2,and 3) based on cardiac function.For e ach group neuropeptide Y level was(112±34),(159±33),and(216±40)ng/L,wit h significant differences(P=0.000 1);NT level in the 3 groups was(99±27),(68 ±30),and(48±15)ng/L,respectively,with highly significant differences(P=0.000 1).Neuropeptide Y level was increased while NT was decreased with the severity of cardiac function.Neuropeptide Y level[(202±43)ng/L] in the patients with le ft ventricular posterior wall and/or interventricular septum hypertrophy was hig her than that of normal thickness group[(125±34)ng/L](t=-9.021 9,P=0.000 1) ,but NT level of hypertrophic group[(52±22)ng/L] was lower than that of the n ormal group[(90±29)ng/L](P=0.000 1). CONCLUSION:Neuropeptide Y and NT participate in the occurrence of essential hy pertension and are related to the damage degree of patients’ cardiac function.Th erefore,evaluation of the changes in neuropeptide Y and NT levels is helpful in assessing the severity and prognosis of essential hypertension.
