超重及单纯性肥胖者血生长激素释放肽变化对自身能量平衡的影响

【摘要】 目的:初步探讨超重及单纯性肥胖者血清生长激素释放肽变化及与生长激素、瘦素相关性。方法:2002-12上海市第一人民医院体检中心就诊患者的超重及单纯性肥胖患者共150例,本文选择超重及单纯性肥胖者共49例作为超重及单纯性肥胖组,男31例,女18例;纳入标准:均为上海地区无亲缘关系单纯性肥胖患者,符合WHO及亚太地区肥胖诊断标准;排除标准:排除由内分泌疾病等引起的继发性肥胖。正常对照组21例,男14例,女7例;均为自愿参加研究、献血的健康的上海市虹口中学教师;纳入标准:正常体质重,无代谢综合征患者。排除标准:排除中心性肥胖、高血压、高脂血症及微量白蛋白尿等患者。用酶标法测定生长激素释放肽、瘦素;放免法测定胰岛素、生长激素。结果:①超重及单纯性肥胖者组血生长激素释放肽水平犤(9.78±2.50)μg/L犦明显低于正常对照组犤(15.07±9.64)μg/L犦(t=2.49,P<0.05);血胰岛素、瘦素水平明显高于正常对照组(t=2.5,3.56,P<0.05,0.01);血生长激素水平明显低于正常对照组(t=3.67,P<0.01)。②血生长激素释放肽与体质量指数呈负相关(t=-0.287,P<0.05),与体质量、身高、三酰甘油、胆固醇及空腹血糖无相关性(P>0.05);与胰岛素、瘦素呈负相关(r=-0.342,r=-0.310,P<0.05);与生长激素无相关性(r=-0.221,P>0.05)。结论

【Abstract】 AIM: To investigate the changes of growth hormone releasing peptide and its correlation with growth hormone and leptin in people with overweight and simple obesity.METHODS: Totally 49 people(31 males and 18 females) with overweight or simple obesity (study group) were selected from the 150 people with overweight or simple obesity, who were treated in the Health Examination Center of First People’s Hospital of Shanghai City.Including criterion: people with simple obesity had no blood relationship with Shanghai area, and were in accordance with the diagnosis criterion of WHO and Asia Pacific area; Excluding criterion: people with secondary obesity induced by endocrine disease.Another 21 subjects (14 males and 7 females), who were blood donor and good health teachers at Hongkou middle school in Shanghai, were taken as the normal controls to their own will; Including criterion: those with normal body mass but without metabolic syndrome; Excluding criterion: patients with central obesity, hypertension, hyperlipemia and trace albumin urine. Serum growth hormone releasing peptide and serum leptin levels were measured by EIA,and the serum insulin and serum growth hormone levels were measured by using radioimmunoassay(RIA).RESULTS:①In the study group, the serum level of growth hormone releasing peptide [9.78±2.50) μg/L] was obviously lower than that in the normal control group[(15.07±9.64) μg/L](t=2.49,P< 0.05);while the serum levels of insulin and leptin were obviously higher than those in the normal control group (t=2.5,3.56,P< 0.05, 0.01), and the serum level of growth hormone was obviously lower than that in the normal control group (t=3.67, P< 0.01). ②Serum growth hormone releasing peptide was negatively correlated with body mass index(BMI)(t=-0.287, P< 0.05), insulin and leptin(r=-0.342, -0.310,P< 0.01), but had no correlation with body mass,height, triglyceride, cholesterol, fasting blood glucose (P >0.05) and growth hormone (r=-0.221,P >0.05). CONCLUSION: Serum growth hormone releasing peptide levels are lower in people with overweight and simple obesity, and the mechanism for its decrease may be a consequence of elevated insulin or leptin, while it has no correlation with the changes of growth hormone.
