原发性高血压患者血清前胶原端肽及生长因子水平与左心室肥厚发生发展的相关性
Relation of procollagen carboxyterminal propeptide and growth factor levels of essential hypertension with the occurrence and development of left ventricular hypertrophy
【摘要】 目的:观察原发性高血压患者血清Ⅰ型前胶原羧基端肽(typeⅠprocolla-gencarboxyterminalpropeptide,PⅠCP)、Ⅲ型前胶原N端肽(typeⅢpro-collagenaminoterminalpropeptide,PⅢNP)、胰岛素样生长因子-I(insulin-likegrowthfactor-I,IGF-I)及转化生长因子-β1(transforminggrowthfactor-β1,TGF-β1)水平的改变,探讨它们在高血压左心室肥厚(leftventricularhypertrophy,LVH)发生发展中的机制。方法:采用放射免疫法测定36例原发性高血压患者(LVH20例,非左心室肥厚(non-leftventricularhypertrophy,NLVH)16例及25例正常对照者血清PⅠCP,PⅢNP,IGF-I和TGF-β1,并对结果进行对比分析。结果:原发性高血压组血清PⅠCP、PⅢNP、IGF-I和TGF-β1的水平(μg/L)分别为162.8±32.7,7.6±3.4,129.5±38.2和40.3±5.4,对照组分别为95.3±13.8,3.8±1.3,68.7±12.5和24.8±4.9,两组比较差异均有显著性意义(t=9.718,5.314,7.661,11.44,P均<0.001),其中LVH组各指标的水平(198.6±35.5,8.9±4.6,143.7±35.8,44.2±6.2)较NLVH组明显升高差异有显著性意义(t=2.474~6.947,P<0.001~0.05)。结论:原发性高血压患者可能有Ⅰ型胶原及Ⅲ型胶原的合成增加、降解相对不足,测定上述各指标在高血压LVH中具有重要的作用。
【Abstract】 AIM:To observe the changes of levels of typeⅠprocollagen carboxyterminal prop eptide(PⅠCP),type Ⅲprocollagen aminoterminal propeptide(PⅢNP),insulin-like g rowth factor-Ⅰ(IGF-Ⅰ) and transforming growth factor-β1(TGF-β1) in patie nts with essential hypertension,and to explore the mechanism of the above parame ters in the occurrence and development of left ventricular hypertrophy (LVH). METHODS:Serum levels of PⅠCP,PⅢNP,IGF-I and TGF-β1 were measured by radio immunoassay in 36 patients with essential hypertension(20 cases of LVH and 16 ca ses of non-LVH) and 25 healthy controls,and comparative analysis was conducted to the results. RESULTS:In the essential hypertension group,the serum levels of PⅠCP,PⅢNP,IG F-I and TGF-β1 were(162.8±32.7),(7.6±3.4),(129.5±38.2) and(40.3±5.4) μg/ L,respectively,significantly different from those in the control group[(95.3±13 .8),(3.8±1.3),(68.7±12.5) and(24.8±4.9) μg/L](t=9.718,5.314,7.661,11.44,all P< 0.001);and the above parameters in the LVH group were obviously increased com pared with the NLVH group(t=2.474-6.947,P< 0.001-0.05). CONCLUSION:PⅠCP and PⅢNP may be the sensitive serum markers of myocardial co llagen synthesis.Myocardial fibrosis may be due to the excessive synthesis and i nsufficient degradation of collagen,and the measurement of the above parameters plays an important role in LVH of essential hypertension.
