亚急性心肌梗死缺血再灌注与心肌细胞活性的实验研究

【摘要】 目的:研究亚急性心肌梗死缺血再灌注与心肌细胞存活关系。方法:16只雄性新西兰大白兔冠状动脉左前降支完全闭塞45min,并再灌注(或无再灌注)7~10d,以18氟-氟代脱氧葡萄糖(18F-fluoro-de-oxyglucose,18F-FDG)为放射自显影示踪剂,三氯四氮唑(triphenyltetra-zoliumchloride,TTC)及HE染色检测梗死灶组织结构改变,透射电镜下观察显影浓聚区内心肌细胞超微结构。结果:无再灌注组,18F-FDG自显影浓聚面与TTC百分比分别为(24.2±1.9)%和(26.5±2.5)%,但差异无显著性意义(t=2.07,P=0.06),而再灌注组两者之间百分比值分别为(20.5±2.4)%和(31.2±2.3)%,差异有显著性意义(t=9.1,P=0.000);电镜下观察到损害严重的心肌细胞仍然摄取18F-FDG。结论:与TTC染色相对照,18F-FDG可反映梗死灶周围心肌细胞存活性,但对病变心肌存活能力的估计过高,尤其表现在再灌注组。

【Abstract】 AIM:To study the relationship between ischemic reperfusion and myocardial viability in rabbits with subacute myocardial infarction. METHODS: Autoradiography with 18F Fluoro Deoxyglucose(18F FDG) was performed in 16 male New Zealand white rabbits,whose left anterior descending coronary artery was completely occluded for 45 min followed by reperfusion(or non reperfusion) for 7- 10 days.The change of infarct tissue construction was determined by triphenyl tetrazolium chloride(TTC) and HE staining.The infarct and periphery myocardial cell ultrastructure were inspected by transmission electron microscopy. RESULTS:In the non reperfusion group, there was no significant difference between the 18F FDG autoradiography density percent[(24.2± 1.9)% ]and the infarct size by TTC staining defected area percent[(26.5± 2.5)% ](t=2.07,P=0.06) , while the two percents[(20.5± 2.4)% and(31.2± 2.3)% ]in the reperfusion group were significantly different(t=9.1, P=0.000).Under the transmission electron microscope,seriously damaged myocardial cells still intook 18F FDG. CONCLUSION:Compared with TTC staining,18F FDG can reflect the periphery infarct myocardial viability,which is overestimate,especially in the reperfusion group.
