血清骨保护素水平与冠状动脉病变的关系

【摘要】 目的:探讨血清骨保护素、血脂与冠状动脉病变严重程度的关系。方法:对90例行冠状动脉造影的患者用酶联免疫吸附试验检测血清骨保护素,同时检测血脂。用冠状动脉病变血管的数量和狭窄度积分来代表冠状动脉病变严重程度。以冠状动脉造影阴性作对照并进行多因素Logistic回归等分析。结果:冠心病组患者血清中骨保护素水平明显高于对照组(F=9.203,P<0.05)。骨保护素水平随冠状动脉病变支数的增加而升高(F=7.571,P<0.001)。冠状动脉正常或内腔轻度不规则、单支病变、2支病变、3支病变的患者,其血清中骨保护素水平(μg/L)分别为0.104±0.035,0.111±0.037,0.130±0.084,0.169±0.077。多元回归分析显示血清中骨保护素与冠状动脉严重程度有密切关系(OR=2.4;95%的可信区间为1.4~4.2)。三酰甘油、低密度脂蛋白胆固醇(LDL-C)是具有统计学意义的冠状动脉病变危险因素。结论:血清中骨保护素水平与冠状动脉的进展可能有直接关系。OPG,LDL-C、三酰甘油水平越高,冠状动脉病变越严重。

【Abstract】 AIM:To explore the relationship of serum osteoprotegerin (OPG) and serum lipid with the severity level and classification of coronary artery lesions. METHODS:Serum OPG was measured by enzyme linked immunosorbent assay(ELISA) in 90 patients who had received coronary angiography (CAG),and lipid level was also measured.The number of diseased vessels and the coronary artery stenosis score were used to show the severity of coronary atherosclerosis lesions.The data were analyzed with logistic regression. RESULTS:Serum OPG levels in patients with coronary heart disease were significantly higher than those in the control group(F=9.203,P< 0.05). As the severity of coronary lesions increased, there was a significant increase in serum OPG levels(F=7.571,P< 0.001).Serum OPG levels were(0.104± 0.035)μ g/L, (0.111± 0.037)μ g/L,(0.130± 0.084)μ g/L,(0.169± 0.077)μ g/L in patients with normal coronary arteries or luminal irregularities,1 vessel disease,2 vessel disease and 3 vessel disease,respectively. Multiple regression analysis showed that serum OPG levels were closely associated with the severity of coronary artery lesions(odds ratio was 2.4; 95% and confidence interval was 1.4- 4.2).Triglyceride (TG) and low density lipoprotein cholesterol(LDL C) were coronary lesion risk factors with statistical significance. CONCLUSION: Serum OPG may be directly related to the initiation and development of coronary artery lesions. The higher the levels of serum OPG, LDL C and TG are, the more serious the degrees of coronary artery lesions will be.
