糖尿病患者文化程度对血糖控制的影响

【摘要】 目的:观察糖尿病患者文化程度与血糖控制的关系。方法:以已经参加过本中心糖尿病教育的患者(n=490)为对象,按其文化程度分为小学(n=53)、初中(n=121)、高中n=(167)和大学(n=149)的4组亚群,观察这些患者在接受糖尿病教育后的血糖控制情况,比较这些糖尿病患者对糖尿病知识问卷的得分,分析不同文化程度的患者对糖尿病知识的理解和掌握情况。结果:这些患者的糖尿病知识问卷得分随着其文化程度的提高逐步升高(F=20.87,P<0.001),进一步采用Student-Neuman-Keuls方法作组间比较,以α=0.05为显著性差异,发现初中明显高于小学(P<0.05),而高中又明显高于初中(P<0.05),高中文化程度以上的人群差异不明显,而小学组与未接受过糖尿病教育的人群得分接近。不同文化程度人群之间,其血糖控制的情况也表现出相似的结果(空腹血糖F=3.90,P<0.01;餐后血糖F=3.95,P<0.01),进一步作组间比较显示高中以上文化程度的患者空腹血糖明显低于小学组和初中组(P<0.05),餐后2h血糖明显低于小学组(P<0.05)。相关分析也发现,空腹和餐后2h血糖均与糖尿病知识问卷的得分呈显著的负相关(r=-0.12,-0.17,P均<0.05)。结论:糖尿病教育的成果与糖尿病患者的文化程度存在一定的关系,文化程度较高的人群对糖尿病知识的理解和掌握较好,血糖控制也较为?

【Abstract】 AIM:To study the relationship between the education levels and blood glucose control in patients with diabetes. METHODS:Four hundred and ninety patients with diabetes, who had all finished the course for diabetic education,were selected from the database of the Diabetes Center of Ruijin Hospital.According to their education levels, the patients were divided into 4 subgroups:elementary school group(n=53), junior high school group(n=121),senior high school group(n=167) and college group(n=149). The blood glucose control after diabetic education of the patients was observed,their scores of questionnaire about diabetic knowledge were compared and the comprehensive levels to the diabetic knowledge in patients with different education levels were analyzed. RESULTS:In these patients, the scores increased accompanied with education levels( F=20.87, P< 0.001) .Furthermore,Student Neuman Keuls test(α =0.05) showed that the scores of the junior high school group were obviously higher than those of the elementary school group(P< 0.05),and those of the senior high school group were obviously higher than those of the junior high school group(P< 0.05),but no significant difference existed between the scores of the senior high school and the college groups,and the scores of the elementary school group were close to those received no diabetic education.Similar situations were observed in blood glucose control among different groups(fasting blood glucose,F=3.90, P< 0.01,postprondial blood glucose,F=3.95, P< 0.01).And further tests showed that the fasting blood glucose in the senior high school and the college groups were obviously lower than those in the elementary school and junior high school groups(P< 0.05),and the postprondial 2 h blood glucose was obviously lower than that in the elementary school group(P< 0.05).Relative analysis showed that fasting and postprondial 2 h blood glucose were significantly and negatively related to the scores of questionnaire about diabetic knowledge(r=- 0.12 and r=- 0.17, P< 0.05). CONCLUSION:There is a certain relation between the result of diabetic education and the education levels of the patients.The higher the education levels are,the better the comprehension to the knowledge is,and the better the blood glucose control is.While to the patients with lower education levels,the diabetic education model should be adjusted and make it more suitable for their understanding in order to make their blood glucose control better.
