呈连续房室结功能曲线的房室结折返性心动过速消融终点

【摘要】 目的 :探讨房室结功能曲线 (AVNFC)呈连续性的房室结折返性心动过速 (AVNRT)射频消融的终点。方法 :80例AVNRT患者分为 3组 :A组 12例 ,各种心房刺激均呈连续性AVNFC ;B组 15例 ,仅心房递增起搏呈跳跃性AVNFC ;C组 53例 ,各种心房刺激均呈跳跃性AVNFC。比较三组患者射频消融前后组内及组间的电生理参数。结果 :消融后三组患者心房递增起搏时最大AH间期 (A1H1max)均比消融前显著缩短 ,A组消融前A1H1max及消融后A1H1max的缩短程度均小于B组和C组 (P <0 .0 5)。B组、C组消融后心房程序刺激时最大AH间期 (A2 H2 /A3H3max)均比消融前明显缩短 (P <0 .0 5) ,但A组消融前后差异无显著性。结论 :对于心房递增起搏和程序刺激均呈连续性AVNFC的AVNRT患者 ,消融后A1H1max的显著缩短可作为消融成功的指标之一。

【Abstract】 Objective: To suggest the endpoint of radiofrequent a blation in atrioventricular nodal reentrant tachycardia (AVNRT) with continuous atrio-ventricular node functional curves(AVNFC).Methods: 80 patients with AVNRT were divided into 3 groups. Group A (n =12): patients with continuous AVNFC during atrial incr emental pacing and extra-stimulations. Group B (n =15): patients with continuous AVNFC during atrial extra-stimulations wi th jumping AVNFC during atrial incremental pacing. Group C (n =53): patients with jumping AVNFC during atrial incremental paci ng and extra-stimulations. Electrophysiological parameters were compared among these groups before and after ablation. Results: The A 1H 1 max were shortened in three groups after ablation. In Group A, The A 1H 1 max before ablation and its decreasing extent after ablation were shorter than those in Group B and C(P<0.05). The A 2H 2/A 3H 3 max were shortened obviously in Group B and C, but there was no significant change in Group A after ablation.Conclusion:Significant shortening of A 1H 1 max could be determined as an indicator for the endpoint of radiofrequent ablation in AVNRT patients with con tinuous AVNFC during atrial incremental pacing and extra-stimulations.
