【摘要】 目的 :评价磁共振胰胆管成像 (MRCP)对肝门部胆管癌 (HC)的诊断作用。方法 :分析总结了26例HC的MRCP表现 ,并与手术病理结果对比。结果 :MRCP对HC梗阻水平的定位准确性达96.2 % ,对HC病变范围确定的符合率为76.9%。总体上HC的MRCP表现包括阻塞、狭窄或充盈缺损。依据大体病理学类型 ,其MRCP表现如下 :息肉型表现为充盈缺损 ;结节型表现为胆管阻塞 ;硬化型多表现为胆管阻塞 ,亦可呈线样狭窄 ;浸润型多呈不规则线样狭窄 ,个别亦可表现为胆管阻塞。结论 :MRCP可反映HC的大体病理学特点 ,但对其定性诊断价值有限。MRCP对病变位置及病灶沿胆管向肝脏方向累及范围的确定较准确 ,对外科手术方案的制订具有重要作用。更多还原

【Abstract】 Objective:To evaluate the diagnostic role of magnetic resonance cholangiopancreatography(MRCP)in hilar cholangiocarcinoma(HC).Methods:MRCP appearances were analyzed and compared with the results of pathology and surgery in26cases with hilar cholangiocarcinoma.Results:MRCP could confirm the level of bile duct obstruction with an accuracy of96.2%.The accordance of MRCP in assessing tumor inˉvolvement was76.9%.The findings on MRCP could be classified as three types which included obstruction,stenosis,and filling defects.According to the gross pathology,MRCP appearances of HC were different among the four types:polypoid type lesion appeared filling defect on MRCP,tubercular type lesion occulted the duct on MRCP,most infiltrating type lesions showed a stenotic duct and only a few with the blocked duct,and the sclerosing type lesion either blocked or narrowed the duct.Conclusion:MRCP could make an accurate diagnosis of the tumor location and the involved extent of the lesion,especially the proximal involved range of the bile ducts.It has an important value in working out the operative regimes for hilar cholangiocarcinoma in general surgery.更多还原
