Ⅰ、Ⅱ期乳腺癌保乳术后的三维适形调强放射治疗

【摘要】 目的 对Ⅰ、Ⅱ期乳腺癌施行保乳术后的病人进行根治性三维适形调强放射治疗,提高局部控制率,减轻放疗副作用,提高了生存质量。方法 2 0 0 4年4月~2 0 0 4年1 0月的1 1例接受过标准保存乳房手术的Ⅰ、Ⅱ期乳腺癌病人,均为女性,年龄31~4 6岁,平均34.7岁。术前触诊肿瘤大小为6mm~30mm ,术后病理结果为:单纯癌4例、浸润性导管癌5例、高分化管状腺癌1例、中分化腺癌1例。手术后三个月内接受三维适形调强放射治疗。乳房切线照射剂量4 7~5 0Gy/ 2 4~31次,局部电子线补量1 0~1 5Gy/ 5~8次。结果 全部病人顺利完成全程放疗,最长1例接受放疗后已经2年半,局部及全身未见复发征象,乳房外观形状良好,放疗反应轻微。结论 通过对Ⅰ、Ⅱ期乳腺癌病人进行三维适形调强放射治疗,达到了提高病人的生存质量,提高肿瘤控制率的目的。

【Abstract】 Objective The aim of three dimensional conformal radiation therapy, is to try to improve the quality of life, decrease the side effect and increase the tumor controlling rate of early-stage breast cancer.Methods From Apr. 2000 to Oct. 2004, 11 patients with early-stage breast cancer were treated by breast-conserving surgery and whole breast irradiation. All the patients were of stage Ⅰ or Ⅱ, and the age was 30 to 46(main 36.6). The dimension of primary tumor was less than 3cm. The three dimensional conformal radiation therapy following breast-conserving therapy is about one month after operation. The total dose of whole breast irradiation was 47~50Gy/24~31f and tangemial fields followed by 10~15Gy boost using suitable-energy electron beams.Results All patients had received irradiation process smoothly with none serious complication..Conclusion It can be concluded that through three dimensional conformal radiation therapy, early-stage breast cancer patients have obtained the better quality of life and ideal tumor controlling rate.
