两种内固定材料在治疗不稳定型转子间骨折中的应用及分析

【摘要】 目的 分析动力髋 (DHS)内固定与解剖钢板内固定治疗不稳定型转子间骨折的手术操作及疗效。 方法 回顾分析 62例不稳定型转子间骨折分别采用动力髋与解剖钢板内固定治疗。其中DHS内固定组 2 7例 ,解剖钢板内固定组 3 5例 ,手术入路相同 ,对比手术时间、术中出血量、骨折解剖复位率、骨折愈合时间、髋关节功能优良率。 结果 解剖钢板内固定治疗手术时间短 (P <0 .0 1) ,术中出血量少 (P <0 .0 5 ) ,骨折解剖复位率高 (P <0 .0 5 ) ,骨折愈合时间无差异(P >0 .0 5 ) ,术后髋关节功能优良率高 (P <0 .0 5 )。 结论 解剖钢板内固定治疗股骨转子间不稳定型骨折优于动力髋 ,手术操作简便 ,术后髋关节功能恢复好 ,是治疗不稳定型股骨转子间骨折较好选择之一。

【Abstract】 Objective To discuss the results and the operative manipulation of two different methods for the treatment of femoral unstable intertrochanteric fractures. Mothods A retrospective analysis was applied to a total of 62 patients suffering from unstable intertrochanteric fractures and were treated with dynamic hip screw (DHS) or anatomic plate. Twenty-seven cases of them were treated with DHS.Thirty-five cases were treated with anatomic plate. The operative approach of two different methods was the same. Compare the operative manipulation and the outcomes of treatment. Results The operative manipulation of anatomic plate internal fixation took less time(P<0.01),lost less blood(P<0.05),had better anatomic reduction status(P<0.05). Better functions of hip are found in the outcomes of treatment(P<0.05). Fractures healing took about the same time after the two kinds of operation (P>0.05). Conclusion The anatomic plate internal fixation is one of the good choices for the treatment of femoral unstable intertrochanteric fractures and it is better than DHS. The operative manipulation is simple and the post-operation functions of hip recover well.
