外环境与医院感染大肠杆菌耐药相关性研究

【摘要】 目的 研究医院感染 (NI)大肠杆菌 (E .coli)与医院环境E .coli之间的相关性 ,以了解NI病原菌来源。 方法 比较E .coli医院感染株 (院感株 )、社区感染株、环境株的耐药谱以及随机引物扩增多态性基因分型 (RAPD)型别之间的差异 ,判断三者之间的相关性。 结果 收集到E .coli院感株 5株 ,社区感染株 17株 ;1190份环境样中共分离到E .coli 72株 ,环境分离率为 6.0 5 %。耐药率、耐药谱与RAPD分型综合研究表明 :在外科院感株和环境株之间有相关性 ,E .coliNI主要是外源性感染 ;其它科室 ,从时间和空间上来看 ,院感株、社区感染株和环境株之间无相关性 ,E .coliNI以内源性感染为主。 结论 在E .coliNI中外源性和内源性途径均存在。

【Abstract】 ObjectiveTo detect the correlation of the E.coli from inpatients with NI, inpatients without NI and from the hospital environment.MethodsTo compare the differences of drug-resistant pathogens and RAPD pathogens between E.coli from inpatients with NI and E.coli from inpatients without NI and the hospital environment so as to determine the correlation between the three pathogens.ResultWe collected 5 strains of colon bacillus from inpatients with NI, 17 strains from inpatients without NI, and from the hospital environment we also isolated 72 strains from 1190 samples. Isolation rate of environment is 6.05%. Study on drug-resistant rate, drug-resistant pathogens and RAPD pathogens indicate that in the surgical department, E.coli from inpatients with NI is related to E.coli from hospital environment, E.Coli NI are mostly heteroinfection. In other department, from time and space point of view, there is no obvious correlation between E.coli from the three sources.ConclusionIn E.coli NI, there exists both heteroinfection and endogenous infection.
