抗精神病药对肝功能的影响

【摘要】 目的 :比较利培酮、氯氮平及氯丙嗪对精神分裂症患者肝功能的影响。方法 :采用日立 70 6 0全自动生化仪对单服利培酮、氯氮平及氯丙嗪治疗的精神分裂症患者各 30例进行治疗前、治疗后 4周及治疗 8周的肝功能各项指标检测对比分析。结果 :利培酮对肝功能不存在差异 (P>0 .0 5 )。氯氮平对肝功能先引起谷丙转氨酶升高 ,后导致谷草转氨酶升高 ,差异存在显著性 (P<0 .0 1)。氯丙嗪对肝功能早期引起谷丙转氨酶升高 ,同时也引起胆红素升高 ,差异亦存在显著性 (P<0 .0 1) ,而第 5周后下降至正常范围。结论 :氯氮平及氯丙嗪均可导致精神分裂患者肝脏代谢异常 ,用氯氮平治疗精神分裂症患者时应及早采取相应的预防措施

【Abstract】 Objective To study the difference in the effect on indices of liver function in schizophrenic patients treated with risperidone,clozapine or chlorpromazine.Method The indices of liver function in schizophrenic patients had been measured and analyzed before and after 4weeks,8weeks using risperidone,clozapine or chlorpromazine in 30 cases respectively.Result There was no significant difference in the liver function caused by risperidone (P>0.05).Alanine aminotransferase levels were significantly higher in the liver function caused by clozapine firstly ,and then asprtate aminotransferase levels were significantly higher.there was a significant difference (P<0.01).Alanine aminotransferase levels were higher in the liver function caused by chlorpromazine,but after 5weeks it was lower to the normal ranges.And while chlorpromazine can also make bilirubin went up,there was also a significant difference (P<0.01).Conclusion It is suggests that a disturbance of liver metabolism exists in schizophrenic patients treated with clozapine or chlorpromazine.It is necessary to take preventive measure in schizophrenic patients treated with clozapine.
