【摘要】 目的　分析探讨鞍上病变的MR诊断和鉴别诊断。方法　 13 6例鞍上病变均经手术病理证实。年龄 3～ 70岁。均行MR平扫检查 ,其中 10 2例行MR增强扫描。分析病变的MR表现。结果　颅咽管瘤 49例 ,脑膜瘤 18例 ,表皮样囊肿 15例 ,生殖细胞瘤13例 ,胶质瘤 8例 ,蛛网膜囊肿 8例 ,错构瘤 5例 ,脂肪瘤 5例 ,Rathke囊肿 5例 ,转移瘤 3例 ,皮样囊肿 3例 ,畸胎瘤 2例 ,组织细胞X1例 ,结节病 1例。通过分析MR表现 ,提出可根据病变信号和组织学特点将鞍上病变分为囊实性病变、T1 WI呈高信号的病变、囊性病变和实质性病变 4大类。结论　将鞍上病变进行分类对其鉴别诊断有重要意义更多还原

【Abstract】 Objective To evaluate MR diagnosis and differential diagnosis of suprasellar diseases .Methods MR plan scans in 136 cases with suprasellar diseases proved pathologically, aged between 3 and 70 years were performed, among which 102 cases were examined by MR contrast scan. Results There were 49 cases of craniopharyngioma, 18 cases of meningioma, 15 cases of epidermoid cyst, 13 cases of germinoma, 8 cases of glioma, 8 cases of arachnoid cyst, 5 cases of hamartoma, 5 cases of lipoma, 5 cases of Rathke’ cleft cyst, 3 cases of metastasis, 3 cases of dermoid cyst, 2 cases of teratoma, 1 case of histiocytoma, 1 case of sarcoid. By analysis of MR manifestations, the suprasellar diseases could be divided into the following four types according to their signal and pathologic features: cystic and parenchymatous lesions, high signal lesions on T 1WI, cystic lesions and parenchymatous lesions. Conclusion It is very significant to make differential diagnosis according to the classification of suprasellar diseases.更多还原
