增强磁共振肺动脉造影诊断肺栓塞的实验研究

【摘要】 目的 探索三维动态增强磁共振肺动脉血管造影 (3DDCEMRPA)诊断急性肺动脉栓塞的价值及限度。方法  6只家兔用明胶海绵栓塞肺动脉 ,在栓塞前及栓塞后分别行 3DDCEMRPA及DSAPA检查 ,栓塞后 2 4~ 72h处死家兔 ,进行病理对照观察。MR所用序列为快速梯度回波 (FFE) ,参数为TR 9ms ,TE 3ms ,SL 2 .5mm ,矩阵 2 56× 12 8,翻转角 2 0° ,对比剂Gd -DTPA 0 .2mmol kg快速注射。结果 病理解剖发现栓子 16个 ,大部分发生在叶动脉水平 ,MRPA诊断肺栓塞的敏感性为 81.2 5% ,特异性为98.65% ;DSAPA的敏感性为 75.0 0 % ,特异性为 98.65%。结论  3DDCEMRPA是一种安全有效的诊断肺栓塞的MRA技术 ,是一种发展前景很可观的诊断方法 ,有很大的发展潜力

【Abstract】 Objective To assess the diagnosis value an d limitation of three-dimensional dynamic contrast-enhanced MR pulmonary angiograp hy in acute pulmonary embolism.Methods Six rabbits were embolized with gelatin sponge su ccessfully. MRPA and DSAPA were performed respectively before and after pulmonar y embolism of each rabbit. These rabbits were anatomized in 24~72 hour after pu l monary embolism. MR sequences were fast field echo(FFE) with 0.2 mmol/kg Gd-DTPA and 2 ml/s fast injected by hand. Scan parameter were TR/TE :9 ms/3 ms, SL 2.5 mm, matrix 256×128 and flip angle 20°.Results 16 embolus were found in dissection. Most of them lie in lobar artery level. The sensitivity of 3D DCEMRPA was 81.25%, specificit y 98.65%; whereas the sensitivity and specificity of DSAPA was 75.00%, 98.65%, r espectively.Conclusion 3D DCEMRPA is a safe and effective MR technolo gy for diagnosis of pulmonary embolism and diagnosis method of good foreground . It has great potential for development.
