小儿急性白血病36例血浆D-二聚体检测及临床意义
The relationship between plasma D-dimer level of the children with acute leukemia and state of the illness
【摘要】 目的 :探讨小儿急性白血病血浆 D-二聚体含量与病情状态的关系。方法 :将 36例急性白血病患儿依据白血病诊疗标准分为初诊组、持续缓解组、复发组 ,采用胶体金方法分别测定其血浆 D-二聚体含量 ,并与对照组进行比较 ,初诊组与其化疗前、后自身对照。结果 :1初诊组、复发组与对照组比较 ,D-二聚体水平明显升高 ( P<0 .0 1 ) ;持续缓解组与对照组比较无明显差异 ( P>0 .0 5 ) ;2初诊组化疗前明显高于化疗完全缓解后 ( P<0 .0 1 )。结论 :1急性白血病患儿在初诊、复发时 D-二聚体水平明显升高 ,完全缓解时降为正常 ,说明急性白血病初诊、复发时存在凝血与纤溶系统激活 ,有继发纤溶亢进 ,随着化疗 ,完全缓解后继发纤溶亢进解除。 2急性白血病 D-二聚体水平与病情状态密切相关 ,可以预测化疗效果。
【Abstract】 Objective:To study the relationship between the plasma D-dimer level of the children with acute leukemia and the state of the illness.Methods:36 children with acute leukemia were divided into three groups according to the diagnosis and treatment standard of acute leukemia ,namely the preliminary diagnosis group,the persistant remission group and the relapes group. The D-dimer level of the 3 groups was examined by means of colloidal gold ,then it was compared with the control group’s D-dimer ,and the preliminary diagnosis group’s was compared with itsef before and after chemotherapy. Results:①Compared with the control group,D-dimer level of the preliminary diagnosis and the relapes group was significant higher(P<0.01),whereas there was no obvious difference in continuing remission group (P>0.05); ①D-dimer level of the preliminary diagnosis was lower after chemotherapy treatment than before chemotherapy. Conclusion:①When the acute leukemia was at the state of preliminary diagnosis and relapes, blood coagulation and fibrinolysis were activated ,secondary hyperfibrinolysis occurred; after chemotherapy. and completely remission.the secondary hyperfibrinolysis was relieved.②There was a close relationgship between plasm D-dimer level and the state of illness ,and plasm D-dimer level could be used as a index in predicting the effect of chemotherapy and the recurrence of acute leukemia.
