【摘要】 目的 :评价纤维支气管镜 (简称纤支镜 )在 ICU呼吸衰竭治疗中的应用价值。方法 :对 62例 ICU住院的呼吸衰竭患者进行纤支镜检查及治疗共 1 5 8例次 ,包括分别在左右主支气管内取痰培养 ,吸痰 ,支气管肺泡灌洗 ,以及对 2 8例患者行纤支镜引导下经鼻气管插管。结果 :62例患者共有 5 6例取得阳性痰培养结果 ,按药敏结果进行治疗 5 2例取得满意疗效 ;46例患者在吸痰后 Sa O2 升高 ,肺部痰鸣明显减少 ;有 3例患者查出支气管肿物 ;2 8例纤支镜引导下经鼻气管插管均一次成功。全部病例未出现危及生命的并发症。结论 :对 ICU住院的呼吸衰竭患者进行纤支镜检查、痰培养、吸痰、支气管肺泡灌洗及纤支镜引导下经鼻气管插管是安全、有效的 ,有重要的临床应用价值。更多还原

【Abstract】 Objective: To investigate the practical value of application of fiber bronchoscope on patients with respiratory failure (RF) in the intensive care unit (ICU). Methods:158 bronchoscopies were performed on 62 hospitalized cases with RF in the ICU, which included sputum aspiration and culture from left and right bronchi, bronchial and alveolar irrigation. Nasal tracheal intubation (NTI) with the aid of fiber bronchoscope was performed on 28 patients. Results: Of 62 patients, 56 cases had positive sputum culture results. 52 cases received satisfactory therapy under the guide of bacterial drug-sensitivity test. 46 cases benefited from sputum aspiration with SaO 2 increased and sputum rales diminished considerably. 3 cases of bronchial cancer were detected. 28 cases of NIT with the aid of fiber bronchoscope were done successfully at the first try. No life-threatening complications occurred in all patients. Conclusion: It is safe, effective and of great clinical value to perform NTI with the aid of fiber bronchoscope and bronchoscopy for the purpose of sputum culture, sputum aspiration and bronchial alveolar irrigation on hospitalized patients with RF in the ICU.更多还原
