小剂量静脉输注胰岛素影响因素分析
FACTORS WHICH INFLUENCE SMALL DOSE INSULIN TRANSFUSION
【摘要】 ①目的 观察静脉输注胰岛素液过程中不同时点及不同输液装置对胰岛素浓度的影响。②方法在瓶装及袋装生理盐水注射液中分别加入 16U/L的诺和灵R或普通胰岛素注射液进行静滴 ,采用化学发光法测定注射前、中、后各 3mL液体中胰岛素浓度。③结果 胰岛素在临床静脉滴注过程中 ,初始 3mL液体中胰岛素的浓度明显高于输液中间和输液末的胰岛素浓度 ,差异有显著性 (F =17.6~ 35 .8,q =6 .17~ 9.36 ,P <0 .0 5 ) ,输液中间阶段、输液末的胰岛素浓度差异无统计学意义 (q =2 .0 7~ 4 .11,P >0 .0 5 )。瓶装与袋装输液装置对胰岛素的浓度无明显影响 (F =1.4 1~ 3.5 2 ,P >0 .0 5 )。在滴注过程中 ,普通胰岛素的浓度较诺和灵R偏低 ,但无统计学意义。④结论 在胰岛素静滴的开始阶段应注意观察病人 ,避免发生低糖血症
【Abstract】 Objective To observe the time and different appliances which affect the concentration of insulin during small dose transfusion. Methods 16 U/L insulin was respectively added into glass bottle and plastic bag for intravenous transfusion. The concentration of insulin was detected in 3 mL of the solution by chemiluminescence at the beginning, middle and at the end of the transfusion. Results The concentration at the beginning was significantly higher than that at the middle and end of the transfusion(F=17.6-35.8,q=6.17-9.36,P<0.05).No significant difference was observed between the middle and the end of the transfusion(q=2.07-4.11,P>0.05). Insulin level was not affected by appliance(F=1.41-3.52,P>0.05), or by different insulin. Conclusion The patients should be observed at the beginning of insulin transfusion to avoid hypoglycemia.
