螺旋CT三维重建图像在测量DDH患儿股骨颈前倾角中的应用
Application of spiral CT three dimensional reconstruction on measurement of femoral neck anteversion
【摘要】 目的 探讨螺旋 CT三维重建技术在发育性髋关节脱位 (DDH)股骨颈前倾角 (FNA)测量中的应用价值。方法 将 30例 DDH患儿的螺旋 CT三维图像旋转获得俯视位和仰视位图像 ,H为股骨头的中心、即股骨头最长径的中点 ,N为股骨颈最狭窄处的中心、即股骨颈最短径的中点 ,HN和检查台面的夹角即为 FNA。记录手术中所测的相应患儿的 FNA。将以上两组数据进行比较及相关分析。结果 三维图像所测 DDH患儿患侧FNA为 38.5°± 5 .9°,术中测量患侧 FNA为 4 0 .2°± 6 .2°;两者比较差异无显著性 (P>0 .0 5 ) ;直线相关分析显示 ,二者有显著相关性 ,r=0 .4 0 1,P<0 .0 5。结论 螺旋 CT三维成像图像接近解剖位 ,测量 FNA直观、准确
【Abstract】 Objective To evaluate the measurement of femoral neck anteversion by spiral CT scan with 3-dimensional reconstruction(3DCT).Methods Radiological assesment was performed by radiography and series spiral CT scan on 30 patients with developmental dislocation of the hip(DDH).The three-dimensional CT images were reconstructed with shaded surface display(3D).The femoral neck anteversion(FNA)of the affected side was measured with 3DCT and in the operation.Results The mean FNA values were 38.5°±5.9° for 3DCT measurement and 40.2°±6.2° for measurement in the operation,the difference between the two values was not significant(P>0.05)but two values were correlative,r=0.401,P<0.05.Conclusion 3DCT measurement of FNA is accurate and effective.
