【摘要】 目的　探讨低剂量血管紧张素转换酶抑制剂 (ACEI)类药对伴血压正常或轻度高血压的 2型糖尿病 (T2 DM)患者尿α1 - MG、β2 - MG的影响。方法　 6 3例 T2 DM患者中 ,血压正常 31例 ,血压轻度升高 32例。随机分成两组 ,治疗组服低剂量 ACEI,对照组服钙离子拮抗剂 ;两照组均含轻度高血压者。比较两组服药前后血压变化 ;分别于服药前及后 16周、 2 4周用放射免疫法 (RIA)测定患者尿 α1 -微球蛋白 (α1 - MG)、β2 -微球蛋白 (β2 -MG)及 2 4 h尿白蛋白 (Alb)。结果　 ACEI类药有明显降低 T2 DM患者尿 α1 - MG、β2 - MG的作用。以降低 α1 - MG的作用更显著 (P<0 .0 1)。其降低 α1 - MG、 β2 - MG及 Alb的作用与其降低血压的作用无明显相关性 (P>0 .0 5 )。结论　低剂量 ACEI类药非常适用于血压正常或轻度升高的 T2 DM患者 ,尤其是α1 - MG升高者 ,且对阻止糖尿病肾病的发生及发展有重要的临床意义更多还原

【Abstract】 Objective To study the effect of low dosage angiotensin converting enzyme inhibitor(ACEI ) -renazepril on α 1-microglobulin(α 1-MG) and β 2-microglobulin(β 2-MG) out of urine of type 2 diabetes millitus(T 2DM)with non-hypertension or hypertesion. Methods 63 Cases of T 2DM patients associated with non-hypertension or hypertesion were divided into 2 group.33 cases were recevied ACEI-cilazepril ,nifedipine groups 30 cases were the controle group,compared the relationship of the variation of the blood pressure of the controle and treatment groups before and after. The α 1-MGand β 2-MG and albuminuria(Alb) were measured by the method of ratioimmunoassay(RIA) drugs before and after 16 weeks and 24 weeks .Results The ACEI-renazepril reduced α 1-MG,and β 2-MG was significant (P<0.01) in T 2DM with non-hypertension or hypertesion,the α 1-MG reduced more significant (P<0.01) compared with the β 2-MG.The reduction of α 1-MG and β 2-MG was not significant (P>0.05) compared with the blood pressure decreased.Conclusion Low dosage renazepril are fited for treating the T 2DM patients with the higher α 1-MG , in spite of the yes-or-non- hypertebsion,and has an effect in preventing of thedevelopment of DN.更多还原
