矽肺结核患者结核分枝杆菌L型感染情况调查

【摘要】 目的 研究矽肺结核患者结核分枝杆菌 L型的感染状况及其临床意义。方法 对矽肺结核组与矽肺组患者进行痰结核分枝杆菌和结核分枝杆菌 L 型培养 ,并采用 IK染色法对培养物作进一步鉴定。结果 矽肺结核组 12 6例患者中 ,结核分枝杆菌培养阳性 18例 ,检出率为 14 % ;结核分枝杆菌 L型培养阳性 6 4例 ,阳性率为5 1% ,两者相比差异极其显著 (P <0 .0 0 1)。且矽肺期别越高 ,痰结核分枝杆菌 L型的阳性检出率也越高 ( 期为38%、 期为 6 7%、 期为 91% ) ,各期患者间差异极其显著 (P <0 .0 0 1)。矽肺组 10 2例患者中 ,结核分枝杆菌培养均为阴性 ,而结核分枝杆菌 L型培养阳性 8例 ,阳性率为 8%。结论 矽肺结核患者结核分枝杆菌 L型的感染率较高 ,开展结核分枝杆菌 L 型检测 ,对减少矽肺结核复发的漏诊或误诊、提高矽肺结核的早期诊断率有着重要的意义。

【Abstract】 Objective To investigate the status of infection caused by L-form of Mycobacterium tuberculosis in patients with pulmonary silico-tuberculosis and evaluate its clinical significance.Methods Sputum culture for Mycobacterium tuberculosis and its L-form was conducted in both silico-tuberculosis and silicosis groups,and IK acid fast smear was used for further identification.Results In silico-tuberculosis group,of total 126 cases,culture positive rate of Mycobacterium tuberculosis was 14% (18 cases),and that of L-form of Mycobacterium tuberculosis was 51% (64 cases),the difference was quite notable (P<0.001).Furthermore,the detected positive rate of L-form ascended following the increase of silicosis stage (stage Ⅰwas 38%,stage Ⅱ was 67%,stage Ⅲ was 91%),the striking difference among various stages was seen(P<0.001).While in silicosis group,none of all 102 cases was found sputum Mycobacterium tuberculosis culture positive,but culture positive rate of its L-form reached 8% (8 cases).Conclusion The infectious rate of L-form of Mycobacterium tuberculosis in patients with pulmonary silico-tuberculosis is much high,it is of important significance to carry out detecting L-form of Mycobacterium tuberculosis,which can diminish mistakes in diagnosis on recurrence of silico-tuberculosis,and facilitate early diagnosis of silico-tuberculosis.
