【摘要】 目的 :探讨口外径路切除舌根良性肿瘤的可行性及优点。方法 :采用咽侧入路及舌骨区入路切除咽喉部良性肿瘤 11例 ,并应用舌瓣后置和 (或 )局部拉拢缝合及胸骨舌骨肌肌筋膜瓣修复舌根创面及缺损。结果 :肿瘤均完整切除 ,随访 3年无复发 ;患者均恢复了饮食及言语功能 ,平均恢复经口进食时间为 12 .0 3d ;患者术后初期均伴有不同程度的言语含糊不清 ,5例切除范围较大者尤为明显 ,经锻炼后逐步好转 ,能恢复正常言语交流。结论 :对口内暴露视野受限 ,肿瘤位置偏后 ,或延及会厌、咽侧壁 ,深部生长明显 ,肿瘤巨大考虑切除后缺损巨大或血运丰富估计经口切除出血难以控制者用口外径路是有效的方法。更多还原

【Abstract】 Objective:To explore the feasibility and advantage to resect the benign tumor of tongue base with the surgery approaches outside oral cavity. Method: 11 patients with benign tumor of tongue base were reviewed , latero-pharynx and regiones hyoidea surgery approaches were performed, and the tongue base defect was repaired with tongue flaps postposition and/or topo-seam, sternohyoideus fascia muscularis flaps. Result: All the patients got successful tumor removed, no recurrence happened after 3 years′ follow-up. The swallow and speech function of all cases recovered successfully, and the mean time to take food with mouth was 12.03 days; speech ambiguity happened at first especially for 5 cases with vast defect and restored after postoperative training. Conclusion:If surgery is restrict view in through oral cavity approach, the tumor is deep and located behind, or extends to epiglottis and/or pharynx side wall, the tumor vegetation is mainly to deepness, the tumor and the defect after excision is large, the tumor has affluent blood supply and bleeding is difficult to control in the operation through oral cavity, the surgery approach outside oral cavity is an available choice.更多还原
