【摘要】 目的 :观察芬太尼复合吗啡硬膜外泵注用于子宫切除术患者的镇痛效果及不良反应。方法 :4 0例择期手术患者 ,ASAI—Ⅱ级 ,随机分为 2组 ,每组 2 0例。A组选用 0 .13%布比卡因 +0 .0 4 %吗啡 +格拉司琼 3mg +生理盐水至10 0毫升 ;B组选用 0 .0 0 3%芬太尼 +0 .0 3%吗啡 +格拉司琼 3mg +生理盐水至 10 0毫升。关腹后先给负荷量A组吗啡1mg +生理盐水至 3毫升 ;B组吗啡 0 .5mg +生理盐水至 3毫升 ,连接镇痛泵以 2ml/h速度泵入镇痛药液进行镇痛。结果 :1、综合镇痛质量两组无明显差异 (P >0 .0 5 ) ;2、两组恶心呕吐、皮肤搔痒等发生率无显著差异 (P >0 .0 5 )。结论 :两组均取得满意的镇痛效果 ,芬太尼复合吗啡配方 ,负荷量及两药剂量均减少 ,副作用无明显增加 ,适合用于术后止痛。更多还原

【Abstract】 Objectives:To observe effect of analgesia and adverse effect of fentanyl combined with morphine by extraduaral pumping for patients with hysterectomy.Methods:We randomly divided 40 cases with hysterectomy from ASAI to ASAII into two groups. A group was given 0.13% bupivacaine plus 0.04% morphine plus 3mg granisetron and plus physiological saline to 100ml. B group was given 0.003% fentanyl plus 0.03% morphine plus 3mg granisetron and plus physiological saline to 100ml. After abdomen was closed, the two groups were first given load capacity, which were 1mg morphine plus physiological saline to 3ml for A group and 0.5mg morphine plus physiological saline to 3ml for B group. Then, the two groups were given analgesia agents by analgesia pump with the speed of 2ml/h. Results:There was no significant difference of general analgesia quality for the two groups (P>0.05). There also were no significant differences of the rates of nausea, vomiting and itching for the two groups (P>0.05). Conclusions:The analgesia effects for the two groups were satisfying. However, for fentanyl combined with morphine, its load capacity and dosage are less and adverse effect does not increase obviously. It is suitable for postoperative analgesia.更多还原
