噬菌体裂解法对涂片阴性肺结核的诊断价值

【摘要】 目的 研究噬菌体裂解法对涂片阴性肺结核的诊断价值。方法 对 15 7例临床诊断为涂片阴性肺结核患者的314份痰标本同时应用噬菌体裂解法和改良罗氏 (L—J)法检测 ,对照分析结果。结果  15 7例涂片阴性肺结核、噬菌体裂解法检出阳性 6 1例 ,阳性获得率 95 .3% (6 1/ 6 4 ) ;L—J法检出阳性 2 8例 ,阳性获得率 4 3.8% (2 8/ 6 4 )噬菌体裂解法和涂片法两者差别有统计学意义 (p <0 .0 1)。 314份涂片阴性疾标本。噬菌体裂解法检测阳性率 36 .9% (116 / 314 ) ,显著高于L—J法 15 .6 %(4 9/ 314 ) ,两者差别有统计学意义 (p <0 .0 1)。噬菌体裂解法检测的报告时间为 2天 ;改良罗氏培养的阳性报告时间平均为2 8.7天。阴性报告时间为 5 6天。结论 噬菌体裂解法检测涂片阴性肺结核。与改良罗氏法相比 ,具有更高的检出率 ,可以简便、快速地检测结核分支杆菌、全程只需 2天 ,对现代结核病控制工作具有重大意义

【Abstract】 Objective To investigate the diagnostic value of Becteriophage Splitting in negative tuberculosis smears. Method Both Becteriophage Splitting and L-J were used to test the 314 sputum samples of 157 cases of negative tuberculosis patients in smear tests. The test results were then contrasted and analyzed. Result Among the 157 cases of negative tubereulosis, 61 cases had positive reaction in Becteriophage Splitting(positive reaction rate=95.3%)while only 28 cases have been Bound by L-J(positive reaction rate=95.3%).Among the 314 sputum samples,36.9%(116/314) were tested positive in Becteriophage Splitting whereas only 15.6%(49/314)were tested positive in L-J. Statistically significant difference was found in both of the contrastive tests(p<0.01).It took L-J 28.7 days to report the positive results and 56 days to report the negative results, a sharp contrast to the reporting time of 2 days in Becteriophage Splitting test. Conclusion compared with L-J, Becteriophage Splitting can test tubercle bacillus more efficiently and quickly in negative tuberculosis smears with a reporting time as short as 2 days. It is of great significance in controlling tuberculosis in modern time.
