【摘要】 目的　探讨儿童股骨颈骨折的治疗方法 ,分析其产生并发症的原因。方法　对 4 6例儿童股骨颈骨折按Colonna分类 ,骨折无移位者采用牵引加髋人字石膏固定治疗 14例 ;有移位者采用牵引加闭合复位多根克氏针固定 16例 ,可吸收螺钉固定 3例 ,术后髋人字石膏固定 ;采用牵引闭合复位空心钉加克氏针或单纯空心钉固定13例 ,术后不用髋人字石膏固定。结果　经 1- 5年术后随访 ,骨折愈合、功能完全恢复 2 8例 ,骨折愈合功能恢复稍差 12例 ,股骨头坏死 3例 ,髋内翻畸形 3例。结论　儿童股骨颈骨折在临床较少见 ,但其并发症、致残率较高 ,预后差。按Colonna分类 ,Ⅰ、Ⅱ型疗效差 ,Ⅲ、Ⅳ型疗效好。无移位骨折保守治疗效果好 ,有移位骨折采用闭合复位、克氏针或螺钉固定效果好 ,愈合率高。更多还原

【Abstract】 Objective To probe the management of the femoral neck fracture(FNF) in children and analyze the causes of its complication,in order to improve the therapeutic effect and reduce its occurrence rate of complication.Methods According to criteria of colonna,46 cases were classified.14 cases of the FNF without displacement underwent traction and hip spica cost plaster.16 cases of the FNF with displacement treated by close reduction and kirscher’s wire internal fixation or hollow screws.After operation plaster external fixation were used.Biofix screw was used in 3 cases without plaster fixation.Results All cases were followed up for 1 year-5 years.The function of 28 cases were totally restored,12 cases was restored with a little unsatisfied function.Avascular necrosis of femoral head was occurred in 3 cases.3 cases complicated with coxa vara.Conclusion The FNF in the children is seldom.But ist occurrence rate of complication is high,prognosis is also bad.According to colonna classification.The effectiveness of type Ⅰ and type Ⅱ are bad,while the one of type Ⅲ and type Ⅳ are good,the cases without displacement can obtain good effect with conservative treatment.The case of FNF with displacement can get better effect by close reduction,internal fixation with kirscher’s pine or screw.更多还原
