甲状腺疾病患者300例甲状腺过氧化物酶抗体的临床变化

【摘要】 目的:观察不同甲状腺疾病以及自身免疫性甲状腺疾病(AITD)不同临床阶段患者血清甲状腺过氧化物酶抗体(TPOAb)浓度的变化,探讨其临床运用价值。方法:采用化学发光法检测300例甲状腺疾病患者血清TPOAb浓度,包括Graves病(GD)120例、桥本病(HT)120例、亚急性甲状腺炎20例、甲状腺结节40例。结果:①不同甲状腺疾病患者血清TPOAb浓度均超过正常值范围,组间存在差异,尤以HT组TPOAb浓度和阳性率最高;②AITD患者不同临床状态血清TPOAb浓度存在动态变化。临床缓解期的GD和HT患者TPOAb浓度均低于疾病初发者,但差异无显著性(P>0.05)。结论:不同甲状腺疾病患者均可出现TPOAb的异常,尤以HT患者显著。TPOAb的降低滞后于临床甲状腺功能的恢复。其临床检测有助于AITD的诊断和疗效评估。

【Abstract】 Objective: To observe the changes of thyroid peroxidase antibody(TPOAb) in patients with thyroid disorders and different clinical phases of autoimmune thyroid diseases(AITD), and evaluate its clinical value. Methods: Serum levels of TPOAb were detected in 300 cases by chemiluminescence immunometric assay including Graves disease(GD), Hashimoto’s disease(HT), subacute thyroiditis and thyroid nodules. Results: ①Serum levels of TPOAb in patients with thyroid disorders were higher than normal range. There existed differences in four groups. The TPOAb levels and positive rates of HT patients were highest. ②TPOAb trended to change in different clinical phases of AITD. The levels of TPOAb in GD and HT remittent groups were lower than that of onset groups, but the difference didn’t reach significance(P > 0.05). Conclusion: Abnormality of TPOAb levels exists in patients with thyroid disorders especially in HT patients. The reduction of TPOAb lags behind the normalization of thyroid function. The detection of TPOAb levels would be helpful to the diagnosis and evaluation of treatment efficacy of AITD.
