P16、P15在子宫颈癌中的表达及临床病理意义研究
THE EXPRESSION AND CLINICAL SIGNIFICANCE OF P 16 AND P15 PROTEINS IN UTERINE CERVICAL CARCINOMA
【摘要】 目的 :研究P16、P15在子宫颈癌中的表达特点 ,并探讨其与临床分期、病理分级、淋巴结转移及预后的关系和意义。方法 :应用超敏S -P免疫组化方法检测 4 3例宫颈鳞癌及 10例正常宫颈组织、10例宫颈上皮内瘤样病变 (CIN)中的P16、P15的表达情况 ,并结合临床分期、病理分级、淋巴结转移及预后情况进行分析。结果 :(1)在 4 3例宫颈癌中P16阳性细胞主要为胞浆染成棕黄色 ,少数标本中胞核也染成棕黄色 ;P15阳性细胞胞核、胞浆均为棕黄染色 ,但胞核染色比胞浆深。 (2 )P16和P15蛋白表达结果相比较无统计学意义 ,但二者在宫颈癌组织的表达明显低于在对照组正常宫颈中和CIN中的表达率。统计学分析有系统性线性趋势。 (3)P16和P15蛋白表达在I、II、III级中的差异均无显著性。 (4)P16、P15在宫颈癌I~IV期中的表达虽有下降趋势 ,但差异均无显著性。 (5 )P16 ,P15在淋巴结转移组中的阳性表达率明显低于在无淋巴结转移组的阳性表达率 ,(6 )子宫颈癌患者术后生存期≥ 5年者 ,P16和P15蛋白的表达阳性率显著高于生存期 <5年者。结论 :抑癌基因P16、P15的表达缺失在子宫颈癌中同时存在 ,二者的表达缺失积极参与了宫颈癌的发生、发展和转移 ,二者的联合表达缺失预示患者预后差。P16、P15蛋白的检测对评估子宫颈癌淋巴结转移
【Abstract】 Objective:The study is to probe the expression and the significance of clinical stage and grading of tumor suppressor genes P16 and P15 in primary cervical carcinomas. Method:Supersensitive S-P immunohistochemical method is used to detect the expression of P16 and P15 proteins in 43 cases of primary cervical squanmous cell carcinomas and 10 cases of normal cervical tissues and 10 cervical intraepithelial neoplasias(CIN). Results:(1)In 43 cases of cervical carcinomas ,positive expression of P16 protein is found mainly in brown cytoplasm, but P15 protein is found in both nucleus and cytoplasm, deep brown. (2) P16 and P15 positive expression rates in cervical carcinomas were significantly lower than those in normal cervixes. (3)No statistical difference of P16 and P15 proteins expression was found between grade I and grade III. (4) In cervical carcinomas, no statistical difference of P16 and P15 proteins expression was found between stage I and stage IV obviously. (5) The positive rates of P16,P15 in cervical carcinomas with lymph node metastasis were significantly lower than those without lymph node metastasis. (6)The positive rates of P16 and P15 proteins in the patients of survival period longer than 5 years were higher then in the shorter than 5 years. Conclusions:There is expressive deletion of proteins of genes P16 and P15 in primary cervical cercinoma .Both descended expression of P16 and P15 may play important roles in the tumorogenesis、progression and malignant behavior in cervical carcinomas.The detection of P16、P15 proteins can be used to evaluate potentiality of lymph node metastasis and also provide the evidence to assess cervical carcinoma prognosis.
