CT灌注成像在兔VX2软组织肿瘤中应用的实验研究

【摘要】 目的:初步评价多层螺旋CT灌注成像方法及其应用价值。方法:10只体重1.6~1.8kg的新西兰大白兔,在其大腿近段注射VX2肿瘤组织悬液0.2ml,利用GElightspeedTM4.XMSCT扫描仪于肿瘤组织接种后第14天行CT平扫和灌注扫描,扫描图像经AW4.0工作站处理,计算并分析灌注图像和灌注参数,包括血流量(BF)、血容量(BV)、平均通过时间(MTT)和表面通透性(PS)。随后将荷瘤兔处死并取出肿瘤,用10%福尔马林固定,于光学显微镜下观察肿瘤组织的结构特点。结果:所有兔大腿VX2肿瘤组织的BF、BV、PS值明显高于正常肌肉组织(P<0.001),而MTT值则明显低于正常肌肉组织(P<0.001)。结论:MSCT灌注成像是一种准确且相对简捷的定量评估肿瘤性病变血流灌注状态的功能成像方法,它可以为肿瘤治疗方案的制定、治疗后的疗效监测以及预后判断提供重要的参考依据。

【Abstract】 Objective: To evaluate the technique and application value of multi-slice spiral CT perfusion imaging preliminarily. Methods: Ten New Zealand white rabbits weighted 1.6~1.8kg were implanted with 0.2ml VX2 tumor tissue suspension in the left or right proximal thigh. Fourteen days later, CT plain scan and perfusion scan were performed on these rabbits respectively by GE lightspeedTM 4.X MSCT scanner, then the images were transmitted to AW4.0 workstation, the functional maps and perfusion parameters including blood flow(BF), blood volume(BV), mean transit time(MTT) and permeability surface(PS) were computed and analyzed. Subsequently, the rabbits were sacrificed and the tumors of which were taken out for normal optical microscopy examination. Results: All the values of BF, BV, PS of rabbits VX2 tumors were obviously higher while the MTT values were lower than that of the normal muscular tissues significantly(P<0.001). Conclusion: MSCT perfusion imaging is an accurate and relatively simple functional imaging technique which can give a quantitative assessment to blood perfusion of tumors, moreover, it can provide important referring basis for making a plan of treatment, monitoring effects after therapy and estimating prognosis.
