【摘要】 目的探讨老年人下肢皮肤癌的修复方法。方法对69例老年人下肢皮肤癌的发病特点及修复方式进行回顾性分析。结果33例游离植皮者,全部成活28例,部分有瘀斑、水疱,换药后愈合3例,因皮片下血肿需再次手术1例,28例皮瓣手术,26例全部成活,2例(1例筋膜皮瓣,1例游离皮瓣)远端有部分瘀斑及坏死,经换药后愈合,8例截肢(趾)伤口均Ⅰ期愈合。结论老年人下肢皮肤癌修复顺序的选择依次为游离植皮、皮瓣转移、截肢(趾)。对皮瓣选择而言,应首选局部皮瓣,其次为筋膜皮瓣、轴型皮瓣、肌皮瓣及游离皮瓣。更多还原

【Abstract】 Objective To study the repair methods of skin cancer in lower extr emity of aged patients. Methods 69 cases with skin cancer of lower limbs of the aged were analyzed retrospectively in its charecteristic and in total survival p rocedures.Results Among 32 free skingrafting cases, there were 28 cases skin gra ft in total survival, 3 cases with some ecchymosis and blister were healed by dr essing change,1 case complicated with hematoma formation needed Once more operat ion.Among 28 cases applied with skin flap, there were 26 cases skin flap in tota l survival, 2 cases(1 with Fascia skin flap,1 with free skin flap) complicated w ith partial distal necrosis and it was healed by dressing change.8 cases underwe nt amputation,and first intention of wound was obtained in all cases. Conclusion The elective order of surgical repair in the elder patients with skin cancer of lower extremities are free skin graft, skin flap, amputation.As for selection o f skin flap,the frist choice is local skin flap, the second is fasciocutaneous f lap, the third is axial pattern skin flap, musculocutaneous flap and free flap.更多还原
