乙二醇、对苯二甲酸对职业接触工人肾脏功能的影响
Nephrotoxic Effects of Etfaylene Glycol Combined with Terephthalic Acid
【摘要】 [目的]采用流行病学调查方法研究乙二醇(EG)、对苯二甲酸(TPA)对职业接触工人肾脏功能的影响。[方法]选择某石化涤纶部TPA操作男工87人和聚酯操作工104人分别为TPA接触组和TPA与EG混合接触组,选取同地区生活习性、经济状况基本相同的无毒物接触史的电厂工人85人为对照组,通过问卷调查和体检收集其既往接触史及健康状况资料,以尿β2-微球蛋白(UBMG)、尿N-乙酰-β-D-氨基葡萄糖苷酶(UNAG)为肾小管损害效应生物标志物,同时以尿微量白蛋白(UMA)、尿转铁蛋白(TRU)和尿免疫球蛋白(IGU)为肾小球损害效应生物标志物。[结果]TPA接触组和TPA、EG混合接触组UBMG、UNAG和TRU、IGU排泄异常率均显著高于对照组。两接触组肾小管、肾小球损害检出率以及肾小管合并肾小球损害检出率均显著高于对照组,混合接触组肾脏功能异常检出率与TPA接触组比较无显著性差异。[结论]TPA接触和EG、TPA混合接触除影响肾小管功能外,还会影响肾小球功能,甚至会引起肾小管和肾小球功能的联合损伤。
【Abstract】 [Objective] We aimed to assess the adverse effects of terephthalic acid(TPA) and/or ethylene glycol(EG)on kidney function with the epidemiological survey on exposed workers. [Methods] 87 workers exposed to TPA in a Dacron department,and 104 workers exposed to both TPA and EC in Polyester department,served as TPA-exposure group and mixture of EC and TPA-exposure group respectively,and 89 controls were selected based on the age and sex.Renal effect biomarkeis(urinary β2-microglobulin,NAG, urinary microalbumin,transferring and immune globulin were measured. [Results] Occupational epidemiological survey showed that the positive rate of UBMG,UNAG,UMA,TRU and IGU increased whether in TPA-expoeure group or mixture of EC and TPA-exposure group. Compared to the control group, the positive rate of UBMG,UNAG, ICU and TRU were significantly higher in TPA-exposure group and mixture group.The prevalence of renal dysfunction was not significandy different in two exposure groups,there was no obvious joint action of EC combined with TPA on renal dysfunction. [Conclusion] EC and TPA exposure could induce the renal dysfunction,not only renal tubule damage,but also glomerular damage.
