【摘要】 目的 :探讨经皮冠状动脉介入治疗 (PCI)前后血浆白细胞介素 8(IL 8)的变化与早期并发症的关系。方法 :观察 12 1例冠心病患者PCI中和PCI后 30d内并发症 ,用ELISA法测定血浆IL 8和血清C 反应蛋白(CRP) ,流式细胞仪测定中性粒细胞表面 β2 黏合素Mac 1(CD11b/CD18)受体表达。结果 :12 1例中 16例出现早期并发症。16例早期并发症患者PCI前、后血浆IL 8、CRP、CD11b/CD18差值水平明显高于无早期并发症患者(均P <0 .0 5 )。随着IL 8、CRP、CD11b/CD18三分层由低到高 ,早期并发症明显增高 (均P <0 .0 5 )。经过多变量Logistic回归分析 ,PCI前IL 8、糖尿病对早期并发症具有独立预测价值。PCI后早期并发症组血浆IL 8差值与CD11b/CD18差值之间呈明显正相关 (r =0 .776 ,P <0 .0 1)。结论 :PCI后早期并发症的发生与PCI前存在的炎性反应程度有关。血浆IL 8水平正常者对PCI后早期并发症具有独立的阴性预测价值。更多还原

【Abstract】 Objective:To investigate the prognostic value of plasma interleukin-8(IL-8) for early complications after percutaneous coronary intervention (PCI). Method:The pre-and post procedural plasma levels of IL-8,serum C-reactive protein(CRP) were examined by immunoassay, and the expression of CD11b/CD18 on neutrophiles was assessed by flow cytometry, while early complications occurred intra-procedure and 30 days after PCI were observed in 121 consecutive patients with coronary heart disease (CHD).Result:Sixteen patients with early complications had a high preprocedural level and the postprocedural differentials of IL-8, CRP and CD11b/CD18 compared to those without complications (all P< 0.05), the occurrence of complications showed a significant increase trend in the patients by tertiles of IL-8, CRP and CD11b/CD18. Pre-procedural levels of IL8 (RR= 5.864, CI= 1.658～ 20.734,P< 0.01), diabetes (RR= 1.587, CI= 1.246～ 2.132, P< 0.05) were the independent predictors of early complications. There were significant correlations between postprocedural differential of IL-8 and CD11b/CD18, (r= 0.776,P< 0.01) in patients with complications.Conclusions:The results reveal that of early complications after PCI contribute to preprocedure inflammatory responses. Normal levels of IL-8 may be the powerful negative predictors of early complications in patients with CHD following PCI.更多还原
